早期社区康复干预高龄脑卒中后患者认知障碍和睡眠障碍的临床疗效评价

摘　　要：目的:分析早期社区康复对高龄脑卒中后患者认知障碍以及睡眠障碍的干预效果。方法:选取2015年3月2日至2016年3月4日福建省龙海市第一医院神经内科收治的脑卒中后高龄患者120例作为研究对象,随机分为观察组和对照组,每组60例。观察组行早期社区康复模式,对照组行基础随访,记录2组护理前后的MMSE分数、Barthel分数、SFMA分数以及睡眠质量。结果:护理前,2组MMSE、Barthel以及SFMA分数比较,差异无统计学意义(P>0.05);护理后,观察组的各项评分[(19.5±3.3)分、(73.2±16.2)分、(79.6±13.2)分]高于对照组[(13.9±3.6)分、(64.1±15.5)分、(71.2±12.7)分],组间比较差异有统计学意义(P<0.05);观察组睡眠质量优良率93.33%高于对照组70.00%,组间比较差异有统计学意义(P<0.05)。结论:高龄脑卒中后患者行社区康复干预模式,有利于认知功能以及睡眠障碍的调节。Objective:To analyze the effects of early community rehabilitation on cognitive impairment and sleep disturbance in elderly patients with stroke.Methods:A total of 120 elderly patients after stroke admitted to the department of neurology of the first hospital of longhai city,fujian province on March 4,2016 were selected as the research objects and randomly divided into observation group and control group,with 60 patients in each group.The observation group received early community rehabilitation mode,and the control group received basic follow-up.The MMSE score,Barthel score,SFMA score and sleep quality before and after nursing care in the two groups were recorded.Results:Before nursing,MMSE,Barthel and SFMA scores of the two groups were compared,and the difference was not statistically significant(P>0.05).After nursing,the scores of the observation group[(19.5±3.3)points,(73.2±16.2)points,(79.6±13.2)points]were higher than those of the control group[(13.9±3.6)points,(64.1±15.5)points,(71.2±12.7)points],and the differences between the groups were statistically significant(P<0.05).The rate of good and good sleep quality in the observation group was 93.33%higher than that of the control group 70.00%,and the difference between the groups was statistically significant(P<0.05).Conclusion:The model of community rehabilitation intervention for elderly stroke patients is beneficial to the adjustment of cognitive function and sleep disorders.
