颞下颌关节盘前移位同颅面矢状向发育间关系的临床研究

摘　　要：目的:研究颞下颌关节盘前移位与颅颌面矢状向发育间的相关性。方法:纳入68例双侧颞下颌关节盘前移位成年正畸患者为关节盘移位组(DD组),27例同期就诊的双侧关节盘位置正常的成年正畸患者为关节盘位置正常组(BN组),采用Winceph9.0软件对所有研究对象的头颅侧位片进行测量,应用SPSS23.0软件进行相关统计学分析。结果:DD组与BN组相比,骨性Ⅱ类患者所占比例较高(P=0.011),矢状向测量指标无性别间差异(P>0.05);DD组相对于BN组,SNB角较小(P=0.004)、上下牙槽座角(P=0.010)偏小,ANB角(P=0.003)、关节角(P=0.015)偏大。随着DD进展颅颌面矢状向骨型的变化将越为显著。结论:DD与颅颌面矢状向发育间存在一定相关性,且无性别间差异,随着DD的进展DD患者骨性Ⅱ类趋势更为显著。Objective: To study the relationship between anterior disc displacement of the temporomandibular joint and the craniofacial sagittal development. Methods: 68 patients were included in the disc displacement group(DD group), 27 patients were included in bilateral normal(BN) group at the same time. Winceph 9.0 software was used to measure cephalometric of all subjects, and the data were analyzed by SPSS 23.0 software. Results: Compared with the BN group, the proportion of class II patients was higher in the DD group(P=0.011), and there was no gender difference in the sagittal measurement indexes(P>0.05). Compared with the BN group, the SNB angle(P=0.004) and the A-B to FH angles(P=0.010) were smaller in the DD group, the ANB angle(P=0.003) and the articulation angle(P=0.015) were larger. As the DD progresses, the changes in the maxillofacial sagittal relationship were more conspicuous. Conclusion: There is a correlation between DD and sagittal development of the craniofacial development, and there is no significant relationship between sexes. The trend of class II is more obvious with the progress of DD.
