摘 要:目的:探讨星状神经节阻滞(SGB)对帕金森病(PD)大鼠模型疼痛的防治作用及其对脑内炎性细胞因子生成的影响。方法:采用脑内左侧纹状体立体定位注射6-羟基多巴胺(6-OHDA)的方法建立大鼠PD模型,术后第1、2、3和4周分别测试各组大鼠双侧后足的机械性刺激缩足阈值(PWT)和热刺激缩足潜伏期(PWL)变化;选取32只PD模型大鼠,随机分为SGB预防组、生理盐水预防组、SGB治疗组及生理盐水治疗组,分别观察连续7 d SGB处理对PD大鼠疼痛的预防作用和治疗作用;采用ELISA法检测大鼠纹状体和中脑导水管周围灰质(PAG)内IL-1β、IL-6和TNF-α的含量变化。结果:PD大鼠术后第2周双侧PWT明显下降且至少维持至术后4周,SGB预防组PD大鼠第2、3和4周双侧PWT显著高于生理盐水预防组,且SGB治疗组PD大鼠第3和4周双侧PWT也较生理盐水治疗组明显增高,差异均有统计学意义,证实SGB可以预防和治疗PD大鼠疼痛;PD大鼠纹状体和PAG内炎性细胞因子IL-1β、IL-6和TNF-α含量明显上升,而连续SGB处理显著减少这些炎性因子的表达。结论:连续SGB可通过减少纹状体和PAG内的炎性细胞因子抑制PD大鼠机械痛敏的发生和维持。Objective: To investigate the preventive and therapeutic effect of stellate ganglion block(SGB) on pain and inflammatory cytokines in experimental Parkinson’s disease(PD) rat model.Methods: PD model was established by intra-striatal injection of 6-OHDA stereotactically.The bilateral hindpaw withdrawal threshold(PWT) and hindpaw withdrawal lantency(PWL) were measured at 1,2,3,and 4 week after intra-striatal injection.32 PD rats were randomly divided into SGB prevention group,saline prevention group,SGB treatment group,and saline treatment group.The preventive and therapeutic effects of repetitive SGB for 7 days on pain in PD rats were evaluated.The contents of IL-1 β,IL-6,and TNF-α in striatum and periaqueductal gray(PAG) were detected by ELISA method.Results: The bilateral PWT in PD model group decreased significantly at the 2 nd week and remained at least until the 4 th week after intra-striatal injection.The PWT of PD rats in SGB prevention group was significantly higher than saline prevention group at 2,3,and 4 week after intra-striatal injection,and also the PWT in SGB treatment group was higher relative to saline treatment group at 3 and 4 week,which indicated that repetitive SGB could prevent and reverse the pain in PD rats.Repetitive SGB could efficiently reduce the up-regulation of IL-1 β,IL-6,and TNF-α in striatum and PAG of PD rats.Conclusion: Repetitive SGB could prevent the occurrence and reverse the maintenance of mechanical pain hypersensitivity in PD rats by reducing the inflammatory cytokines in striatum and PAG.
