结直肠癌患者CD4^+、CD8^+T淋巴细胞水平对预后的影响

摘　　要：目的探讨CD4^+、CD8^+T淋巴细胞在结直肠癌患者中的表达及对预后的影响。方法选择2013年1月-2015年12月内江市第一人民医院收治的219例结直肠癌患者为研究对象,比较其结直肠癌组织、癌旁组织及正常组织CD4^+、CD8^+T淋巴细胞水平,并分析CD4^+、CD8^+T淋巴细胞水平与患者临床病理特征的关系。随访3年,收集患者相关临床资料进行预后的单因素及多因素分析。结果结直肠癌组织CD4^+、CD8^+T淋巴细胞水平明显高于癌旁组织及正常组织,差异有统计学意义(P<0.05)。有无淋巴转移、不同TNM分期在CD4^+水平中差异有统计学意义(P<0.05);有无淋巴转移、不同TNM分期、癌胚抗原(CEA)在CD8^+水平中差异有统计学意义(P<0.05)。经单因素分析,有无转移、TNM分期、组织学分型、CEA、治疗方式、CD4^+、CD8^+T淋巴细胞为患者预后的影响因素(P<0.05)。经多因素分析,患者TNM分期、转移、CD8^+、CD4^+为影响患者预后的独立影响因素(P<0.05)。结论 CD4^+、CD8^+T淋巴细胞水平在患者癌组织和癌旁组织中存在差异,TNM分期、转移、CD8^+、CD4^+为影响患者预后的独立影响因素,可为临床诊断、治疗提供依据。Objective To explore the expression on the levels of CD4^+,CD8^+T lymphocytes in patients with colorectal cancer and its prognostic significance.Methods 219 patients with colorectal cancer admitted to our hospital from January 2013 to December 2015 were enrolled for the study.The levels of CD4^+,CD8^+T lymphocytes in colorectal cancer tissues,paracancerous tissues and normal tissues were compared.The relationships between the levels of CD4^+,CD8^+T lymphocytes and the clinicopathological features of the patients were analyzed.Follow-up for 3 years,the clinical data of the patients were collected and the prognosis was analyzed by univariate and multivariate analysis.Results The levels of CD4^+and CD8^+T lymphocytes in colorectal cancer tissues were significantly higher than those in adjacent tissues and normal tissues(P<0.05).The presence or absence of lymphatic metastasis and different TNM stages had statistically significant differences in CD4^+levels(P<0.05).The presence or absence of lymphatic metastasis,different TNM stages,and carcinoembryonic antigen(CEA) had statistically significant differences in CD8^+levels(P<0.05).Through single factor analysis,the presence or absence of metastasis,TNM staging,histological typing,CEA,treatment method,CD4^+and CD8^+T lymphocytes were the influencing factors for the prognosis of patients(P<0.05).After multivariate analysis,the TNM stage,metastasis,CD8^+and CD4^+T lymphocytes were independent influencing factors(P<0.05).Conclusion The levels of CD4^+and CD8^+T lymphocytes in patients with cancer tissues and adjacent tissues were different;TNM staging,metastasis,CD8^+,CD4^+T lymphocytes were independent factors affecting the prognosis of patients,and could provide a basis for clinical diagnosis and treatment.
