摘 要:目的探讨阴道微生态相关指标与高危型人乳头瘤病毒(HR-HPV)感染的关系。方法选取2016年4月-2017年4月于东莞市人民医院就诊的HR-HPV患者80例,所有患者均为HR-HPV阳性,将其作为阳性组。选取同期于本院妇科进行体检的体检者80名(HR-HPV均为阴性)作为阴性组。比较两组阴道微生态指标,包括阴道pH值、乙酸氨基β-半乳糖苷酶浓度、白细胞酯酶浓度、唾液酸苷酶浓度、过氧化氢浓度、阴道清洁度、WBC,分析阴道微生态环境与HR-HPV的关系。经Logistic回归模型对有统计学意义的变量进行量化赋值,观察各变量与HRHPV的关系。结果阳性组乙酸氨基β-半乳糖苷酶阳性、白细胞酯酶阳性、唾液酸苷酶阳性、过氧化氢阳性、阴道清洁度异常、WBC阳性占比分别为42.50%、41.25%、37.50%、38.75%、42.50%、56.25%,显著高于阴性组18.75%、23.75%、15.00%、17.50%、20.00%、32.50%,差异均有统计学意义(P<0.05)。Logistic回归性分析提示乙酸氨基β-半乳糖苷酶阳性、白细胞酯酶阳性、唾液酸苷酶阳性、过氧化氢阳性、阴道清洁度异常与HR-HPV感染密切相关(P<0.05)。结论与HR-HPV阴性者相比,HR-HPV阳性者的阴道微生态指标明显异常,且阴道清洁度异常率增高,这些微生态指标与HR-HPV密切相关。Objective To explore the relationship between vaginal microecology and high-risk human papillomavirus(HR-HPV) infection.Methods Eighty patients with HR-HPV treated in our hospital from April 2016 to April 2017 were selected.All patients were HR-HPV positive.They were regarded as positive group.Eighty gynecologic examinees(all HR-HPV negative) who underwent gynecological examination in our hospital during the same period were selected as negative group.The vaginal microecological indicators of the two groups were compared,including vaginal pH value,amino-betagalactosidase acetate concentration,leucocyte esterase concentration,sialidase concentration,hydrogen peroxide concentration,vaginal cleanliness and the relationship between vaginal microecological environment and HR-HPV was analyzed.Logistic regression model was used to quantify the variables with statistical significance,and the relationship between the variables and HR-HPV were observed.Results The positive group had 42.50%,41.25%,37.50%,38.75%,42.50% and 56.25% acetamidobeta-galactosidase,leukocyte esterase,sialidase,hydrogen peroxide,abnormal vaginal cleanliness and WBC,respectively,which were significantly higher than those of negative group(18.75%,23.75%,15.00%,17.50%,20.00%,32.50%,respectively).The differences were significant(P<0.05).Logistic regression analysis showed that positive acetamidobeta-galactosidase,leukocyte esterase,sialidase,hydrogen peroxide and abnormal vaginal cleanliness were closely related to HR-HPV(P<0.05).Conclusion Compared with HR-HPV negative patients,the vaginal microecological indicators of HR-HPV positive patients were significantly abnormal,and the abnormal rate of vaginal cleanliness was increased.The vaginal microecological indicators were closely related to HR-HPV.
