运动针刺后溪穴联合止痛消炎软膏治疗落枕疗效观察

摘　　要：目的观察运动针刺后溪穴联合止痛消炎软膏治疗落枕的临床疗效。方法选取2018年1月至2019年10月在海南省人民医院针灸科门诊就诊的60例落枕患者为研究对象,按照随机数表法分为观察组和对照组,每组30例,观察组给予运动针刺后溪穴加外敷止痛消炎软膏治疗,对照组给予口服塞来昔布加外敷止痛消炎软膏治疗,两组患者均治疗3 d。于治疗前后检测并比较两组患者的血清超敏C反应蛋白(hs-CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平、疼痛视觉模拟评分(VAS)、颈椎功能障碍指数(NDI)及临床效果。结果观察组患者的治疗总有效率为90.0%,明显高于对照组的70.0%,差异有统计学意义(P<0.05);治疗后,观察组和对照组患者的VAS评分[(1.72±0.78)分vs(2.64±1.57)分]、NDI评分[(5.05±1.46)分vs(12.34±2.55)分]比较,观察组明显低于对照组,差异均有统计学意义(P<0.05);治疗后,观察组和对照组患者的血清hs-CRP[(4.47±1.34)mg/L vs(7.37±2.11)mg/L]、IL-6[(43.26±10.15)pg/L vs(76.84±13.36)pg/L]、TNF-α[(0.78±0.41)μg/L vs(1.74±0.35)μg/L]水平比较,观察组明显低于对照组,差异均有统计学意义(P<0.05)。结论运动针刺后溪穴联合外敷止痛消炎软膏治疗落枕能很快改善临床症状,血清炎症因子明显下降,疗效满意。Objective To observe the clinical effect of acupuncture therapy coordination movement at Houxi Point and Zhitong Xiaoyan Oinment in the treatment of stiff neck.Methods A total of 60 cases of stiff neck patients were selected from the outpatient Department of Acupuncture in Hainan General Hospital from January 2018 to October 2019.According to random number table method,the patients were divided into the observation group and control group,with 30 patients in each group.The observation group was treated with acupuncture therapy coordination movement at Houxi Point and Zhitong Xiaoyan Oinment,and the control group received oral celecoxib and Zhitong Xiaoyan Oinment.Patients in both groups were treated for 3 days.The visual analogue score(VAS)and neck disability index score(NDI)of the two groups were observed before and after treatment.At the same time,serum high-sensitivity C-reactive protein(hs-CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)levels were detected,and the clinical effects of the two groups were compared.Results The total effective rate in the observation group was 90.0%,which was significantly higher than 70.0%in the control group(P<0.05).After treatment,the VAS score and NDI score in the observation group were(1.72±0.78)points and(5.05±1.46)points,which were significantly lower than corresponding(2.64±1.57)points and(12.34±2.55)points in the control group(P<0.05);the serum hs-CRP,IL-6,TNF-αof the observation group were(4.47±1.34)mg/L,(43.26±10.15)pg/L,(0.78±0.41)μg/L,which were significantly lower than corresponding(7.37±2.11)mg/L,(76.84±13.36)pg/L,(1.74±0.35)μg/L of the control group(all P<0.05).Conclusion The treatment of stiff neck with acupuncture therapy coordination movement at Houxi Point and Zhitong Xiaoyan Ointment can quickly improve symptoms,reduce serum inflammatory factors,with satisfactory clinical efficacy.
