摘　　要：目的探讨女性甲状腺乳头状癌单侧腺叶及峡部切除术围手术期嗓音声学参数的变化特点。方法回顾性选取2018年1月至2020年1月首都医科大学附属北京友谊医院收治的甲状腺乳头状癌女性患者37例作为试验组,在甲状腺乳头状癌单侧腺叶及峡部切除术前及术后1周内进行电子喉镜检查和多维嗓音声学分析;另在该院门诊选取40例正常女性作为对照组。比较两组研究对象手术前后相关声学参数,分析指标包括平均基频(F 0)、最高基频(hi)、F 0标准差(STD)、基频微扰(Jitter)、振幅微扰(Shimmer)、噪/谐比(NHR)、嗓音扰动指数(VTI)、次谐波度(DSH)。结果试验组和对照组的术前各声学指标比较,差异无统计学意义(P>0.05),说明术前患者嗓音质量与正常组无差异;试验组术后1周与术前比较,F 0、F hi、F 0标准差(STD)、基频微扰(Jitter)、振幅微扰(Shimmer)、噪/谐比(NHR)、嗓音扰动指数(VTI)以及次谐波度(DSH)均显著降低,差异具有统计学意义(P<0.05),与对照组比较,上述声学参数也显著减低,差异具有统计学意义(P<0.05)。结论甲状腺乳头状癌女性患者在单侧腺叶及峡部切除术后,短期内(1周内)嗓音声学客观指标会出现异常。Objective To investigate the changes of acoustic parameters during perioperative period in female patients with papillary thyroid carcinoma underwent unilateral lobectomy plus isthmectomy.Methods This study was a retrospective study.Thirty-seven female patients with papillary thyroid carcinoma admitted to Beijing Friendship Hospital,Capital Medical University from January 2018 to January 2020 were selected as the experimental group.videolaryngoscopy and multidimensional acoustic voice analysis were performed before and one week after the unilateral lobectomy plus isthmectomy.40 normal women in our outpatient center were selected as the control group,and acoustic parameters of the two groups were compared,including fundamental frequency(F 0),highest fundamental frequency(F hi),F 0 standard deviation(STD),fundamental frequency perturbation(Jitter),amplitude perturbation(Shimmer),Noise/harmonic ratio(NHR),voice disturbance index(VTI)and subharmonic degree(DSH).Results There was no statistically significant difference in preoperative acoustic parameters between the experimental group and the control group(P<0.05),indicating that preoperative voice quality of the patients was not different from that of the normal group.Compared with preoperative,the F 0,F hi,STD,Jitter,Shimmer,NHR,VTI and DSH of the experimental group were significantly reduced one week after surgery,and the difference was statistically significant(P<0.05).Compared with the control group,the above acoustic parameters were also significantly reduced,and the difference was statistically significant(P<0.05).Conclusion In female patients with papillary thyroid carcinoma after unilateral lobectomy plus isthmectomy,the objective acoustics voice parameters will be abnormal in a short period of time(1 week).
