摘 要:目的分析2型糖尿病患者外周血单个核细胞(PBMC)中微小RNA-181a(miR-181a)、微小RNA-9(miR-9)的表达及其与胰岛素抵抗的关系。方法选取2018年12月—2019年12月延安大学附属医院内分泌代谢科诊治2型糖尿病患者97例作为观察组,常规体检者93例作为健康对照组。比较2组空腹胰岛素(FINS)及胰岛素抵抗指数(HOMA-IR),空腹血糖(FPG)、餐后2 h血糖(2hPG)、糖化血红蛋白(HbA1c)及PBMC中miR-181a、miR-9的表达差异,并分析2型糖尿病患者胰岛素抵抗与上述相关指标的相关性。结果观察组FINS、HOMA-IR、FPG、2 hPG及HbA1c水平均明显高于健康对照组(t=17.808、14.601、14.182、14.744、17.154,P均=0.000);观察组PBMC中miR-181a、miR-9的表达量均明显高于健康对照组(t=10.476、24.113,P均=0.000);Pearson法分析显示,在2型糖尿病患者中,HOMA-IR与miR-181a、miR-9表达均呈正相关(r=0.413、0.464,P均=0.000);Logistic回归分析显示,BMI、2hPG、HbA1c、miR-181a及miR-9表达均是2型糖尿病患者发生胰岛素抵抗的危险因素[OR(95%CI)=1.97(1.47~2.63)、1.99(1.37~2.88)、2.31(1.42~3.77)、2.72(1.58~4.69)、2.53(1.66~3.87)]。结论miR-181a、miR-9在2型糖尿病患者外周血PBMC中表达上调,且二者均与HOMA-IR呈正相关。Objective To study the expressions of microRNA-181a(miR-181a)and microRNA-9(miR-9)in peripheral blood mononuclear cells(PBMC)of type 2 diabetes mellitus and their relationships with insulin resistance.Methods From December 2018 to December 2019,97 patients with type 2 diabetes diagnosed and treated by the Department of Endocrinology and Metabolism of the Yan'an University Affiliated Hospital were selected as the observation group,and 93 normal healthy people who had routine physical examination in the same period were selected as the control group.The FINS and HOMA-IR,FPG,2hPG,HbA1c and kidney functions,and the expressions of miR-181a and miR-9 in PBMC were compared between the two groups,and the relationship between insulin resistance and the related indexes was analyzed.Results The levels of FINS,HOMA-IR,serum FPG 2hPG and HbA1cin the observation group were significantly higher than those in the control group(t=17.808,14.601,14.182,14.744,17.154,P=0.000);The expressions of miR-181a and miR-9 in PBMC of the observation group were significantly higher than those of the control group(t=10.476,24.113,P=0.000);Pearson analysis showed that HOMA-IR was positively correlated with the expressions of miR-181a and miR-9 in PBMC of patients with type 2 diabetes(r=0.413,0.464,P=0.000);Logistic regression analysis showed that BMI,2hPG,HbA1c,miR-181a expression and miR-9 expression were risk factors of insulin resis[OR(95%CI)=1.97(1.47~2.63),1.99(1.37~2.88),2.31(1.42~3.77),2.72(1.58~4.69),2.53(1.66~3.87)].Conclusion The expressions of miR-181a and miR-9 are up-regulated in peripheral blood PBMC of type 2 diabetic patients,and both of them are positively correlated with HOMA-IR.
