摘　　要：目的探讨肾上腺少见肿瘤MSCT特征,提高肾上腺少见肿瘤的诊断及鉴别诊断水平。方法回顾性分析20例经临床手术病理证实的肾上腺少见肿瘤,观察其MSCT表现特征。结果 20例中,肾上腺皮质癌3例,体积较大,增强扫描渐进性强化,可见腹膜后淋巴结和对侧肾上腺转移等恶性征象;神经母细胞瘤5例,内见钙化,增强扫描实性部分不均匀中度强化;恶性神经鞘瘤1例,增强扫描动脉期、静脉期肿块实性成分及边缘中度强化,其内部分低密度区未见强化;淋巴瘤1例,增强扫描持续强化,肿块包绕周围血管,伴有腹膜后淋巴结增大;节细胞神经瘤2例,静脉期及平衡期肿块内见线条状逐渐强化影;海绵状血管瘤1例,囊性成分为主,内见钙化,其中见出血;肾上腺囊肿3例,密度均匀,增强扫描未见强化;髓样脂肪瘤4例,内见脂肪密度,增强扫描脂肪密度未见强化,实性成分轻度强化。结论肾上腺少见肿瘤各具有特征性MSCT表现特点,MSCT具有一定的诊断及鉴别诊断价值。Objective To investigate the MSCT features and to improve the level of MSCT diagnosis and differential diagnosis of rare adrenal neoplasms.Methods A retrospective analysis of 20 cases of adrenal rare tumors confirmed by clinical operation and pathology was conducted to observe their MSCT characteristics.Results Among the 20 cases of rare adrenal neoplasms,there were adrenocortical carcinomas in 3 cases which had larger volume and progressive enhancement with enhanced medicament,and had malignant signs like retroperitoneal lymph node and contralateral adrenal gland metastasis.There were neuroblastomas in 5 cases with calcification inside,which had moderate enhancement in solid part on enhanced scan.There was 1 case of malignant schwannoma,which had moderate enhancement in enhanced scanning of solid components of arterial and venous mass and moderate edge,and no enhancements in some low density areas were found.1 case of lymphoma,with continuous enhancement of enhanced scan,was surrounded by peripheral blood vessels,and accompanied by retroperitoneal lymph node enlargement.In 2 cases of ganglioneuroma,linear shadow was gradually enhanced in the mass at venous and equilibrium stages.1 case of cavernous hemangioma was found,which mainly has cystic component with calcification and hemorrhage.3 cases of adrenal cyst showed uniform density and no enhancement in enhanced scan.There were 4 cases of medullary lipoma with fat density inside,but no enhancement was found in the enhanced scanning,and the solid component was slightly enhanced.Conclusion The MSCT of rare adrenal neoplasm have their own characteristics,MSCT is helpful in diagnosis and differential diagnosis for the rare adrenal neoplasms.
