摘 要:目的探讨缺氧诱导因子1α(HIF-1α)、上皮钙粘附蛋白(E-cadherin)在肾透明细胞癌组织、癌旁组织中的表达情况,以及其临床意义。方法收集自2017年11月至2019年7月于北部战区总医院和平院区泌尿外科行肾切除术并术后病理诊断为肾透明细胞癌的52例标本。另选取20例距癌组织外缘5.0 cm处正常组织作为对照。采用免疫组织化学染色法检测HIF-1α、E-cadherin的表达情况。分析HIF-1α、E-cadherin与临床病理特征之间的关系。采用Spearman相关性分析法分析HIF-1α与E-cadherin表达的相关性。结果HIF-1α在肾透明细胞癌组织的阳性表达率为69.20%(36/52),高于癌旁组织的25.00%(5/20),差异有统计学意义(P<0.05);E-cadherin在肾透明细胞癌组织的阳性表达率为32.69%(17/52),低于癌旁组织的85.00%(17/20),差异有统计学意义(P<0.05)。HIF-1α、E-cadherin的表达水平与患者肿瘤的病理学分级、临床病理分期具有相关性(P<0.05)。Spearman相关性分析显示,肾透明细胞癌组织中HIF-1α与E-cadherin表达呈负相关(r=-0.601,P<0.05)。结论在肾透明细胞癌中,HIF-1α呈高表达,E-cadherin呈低表达,两者可能成为判断肾细胞癌生物学行为的有效指标。Objective To investigate the expression of hypoxia-inducible factor 1α(HIF-1α)and E-cadherin in clear cell renal cell carcinoma(ccRCC)and adjacent normal tissues.Methods Fifty-two specimens of clear cell carcinoma of the kidney were collected from November 2017 to July 2019 after nephrectomy and pathologic diagnosis.Another 20 patients with normal tissue at 5.0 cm from the outer edge of the tumor tissue were selected as controls.HIF-1αand E-cadherin expression were examined by immunohistochemical staining.The correlation between HIF-1α,E-cadherin and clinicopathological features was analyzed.Spearman correlation analysis was used to analyze the correlation between HIF-1αand E-cadherin expression.Results The positive expression rate of HIF-1αin the renal clear cell carcinoma tissues was 69.20%(36/52),which was higher than that in the adjacent tissues(25.00%,5/20),indicating a statistically significant difference(P<0.05).The positive expression rate of E-cadherin in renal clear cell carcinoma tissues was 32.69%(17/52),which was lower than that in paracancer tissues(85.00%,17/20),P<0.05.The expression levels of HIF-1αand E-cadherin were correlated with the pathological grading and TNM stage of the tumor(P<0.05).Spearman correlation analysis showed that HIF-1αliming was negatively correlated with E-cadherin expression in clear renal cell carcinoma tissues(r=-0.601,P<0.05).Conclusion In renal clear cell carcinoma,HIF-1αis highly expressed and e-cadherin was poorly expressed,both of which may be effective indicators to judge the biological behavior of renal cell carcinoma.
