摘　　要：目的探讨经皮肝穿刺胆道内支架植入术(PTIBS)对恶性胆道梗阻患者免疫学指标的影响。方法选取2016年2月—2018年6月秦皇岛市第一医院收治的124例恶性胆道梗阻患者,随机分为PTIBS组和经皮肝穿刺胆道引流术(PTCD)组,每组62例。比较2组患者的临床疗效及免疫学指标水平。结果PTIBS组总有效率显著高于PTCD组(95.16%vs.72.58%,P<0.05)。治疗前2组CD4^+、CD8^+、CD4^+/CD8^+差异无统计学意义(P>0.05);治疗后PTIBS组CD4^+、CD8^+、CD4^+/CD8^+显著高于PTCD组(P<0.05)。治疗前2组IgA、IgM、IgG水平差异无统计学意义(P>0.05),治疗后PTIBS组IgA、IgM、IgG水平均显著低于PTCD组(P<0.05)。结论治疗恶性胆道梗阻的过程中,PTIBS在疗效、提高患者细胞免疫功能等方面均优于PTCD组,具有良好的临床应用价值。Objective To investigate the effect of percutaneous transhepatic insertion of biliary stent(PTIBS)on the immunological indexes of patients with malignant biliary obstruction.Methods From February 2016 to June 2018,124 patients with malignant biliary obstruction admitted to Qinhuangdao First Hospital were randomly divided into PTIBS group and percutaneous transhepatic cholangial drainage(PTCD)group,62 cases in each group.The clinical efficacy and immunological indexes between the two groups were compared.Results The total effective rate of the PTIBS group was significantly higher than that of the PTCD group(95.16%vs.72.58%,P<0.05).There was no significant difference in CD4^+,CD8^+,CD4^+/CD8^+between the two groups before treatment(P<0.05).After treatment,CD4^+,CD8^+and CD4^+/CD8^+in the PTIBS group were significantly higher than those in the PTCD group(P<0.05).There was no significant difference in the levels of IgA,IgM and IgG between the two groups before treatment(P>0.05).After treatment,the levels of IgA,IgM and IgG in the PTIBS group were significantly lower than those in the PTCD group(P<0.05).Conclusions In the treatment of malignant biliary obstruction,PTIBS is superior to PTCD group in therapeutic effect and improvement of cellular immune function,which has a good clinical application value.
