摘  要:目的:通过光学相干断层扫描血管成像(OCTA)和黄斑功能评估仪(MAIA)微视野计分别测定高、中、低度近视及正视者视网膜黄斑区微循环及黄斑区视功能,并分析其相关性。方法:横断面研究。选择2019年2─8月至南昌爱尔眼科医院就诊的患者及志愿者共112例(112眼),其中高度近视组(A组,屈光度≤-6.00 D)46例、中度近视组(B组,-6.00 D<屈光度≤-3.00 D)37例、低度近视及正视组(C组,-3.00 D<屈光度≤+0.50 D)29例。使用OCTA和MAIA微视野计分别测定各组视网膜黄斑区微循环及黄斑区视功能。采用单因素方差分析比较3组间各OCTA参数及各MAIA微视野计参数,采用Pearson相关分析对视网膜黄斑区微循环与黄斑功能的相关性以及各OCTA参数、各MAIA微视野计参数与眼轴长度(AL)的相关性进行分析。结果:A组、B组、C组深层黄斑区毛细血管密度(MVD)分别为44.5%±6.6%、47.2%±7.4%、48.4%±5.3%;3组总体差异有统计学意义(F=3.449,P=0.035);两两比较中,A组低于C组,且差异有统计学意义(P=0.015),A组与B组、B组与C组比较差异无统计学意义。3组浅层MVD、黄斑中心凹无血管区(FAZ)面积比较,差异均无统计学意义。A组、B组、C组视网膜光敏感度(RS)分别为(27.7±2.2)dB、(28.0±1.8)dB、(29.2±1.0)dB,3组间比较差异有统计学意义(F=5.816,P=0.004);两两比较中,A组低于C组,B组低于C组,差异有统计学意义(P=0.001、0.015);A组和B组比较差异无统计学意义。A组、B组、C组黄斑完整性指数(MI)分别为47.7±31.0,40.9±27.2,25.1±22.6,3组间总体差异有统计学意义(F=5.918,P=0.004);两两比较中,A组高于C组,B组高于C组,差异有统计学意义(P=0.001、0.024);A、B组间差异无统计学意义。3组固视稳定性(FS)指标(P1、P2、63%BCEA、95%BCEA)及检测时间(t)比较差异均无统计学意义。浅层MVD与RS无相关性(r=0.184,P=0.052),深层MVD与RS呈正相关(r=0.281,P=0.003),FAZ面积与RS呈负相关(r=-0.277,P=0.003)。Objective:The optical coherence tomography angiography(OCTA)and macular function evaluator(MAIA)microperimeter were used to measure the microcirculation and macular visual function in high myopia,moderate myopia,low myopia and ocular microscopy.Measurements were analyzed for their relevance.Methods:This was a cross-sectional study.A total of 112 patients(112 eyes)who were treated at Nanchang Aier Eye Hospital from February 2019 to August 2019 were selected,of which 46 patients(46 eyes)with high myopia were in group A(refractive power≤-6.00 D),37 patients(37 eyes)with moderate myopia were in group B(-6.00 D<refractive power≤-3.00 D),and 29 patients(29 eyes)with low myopia and emmetropia were in group C(-3.00 D<refractive power≤+0.50 D).OCTA and MAIA microperimetry were used to measure the microcirculation and macular visual function of the retina in each group.Univariate analysis of variance was used to compare the OCTA parameters and MAIA microperimeter parameters among the three groups.The correlation between regional microcirculation and macular function,and the correlation between various OCTA parameters,and each MAIA microperimeter parameter and AL were analyzed.Results:The deep microvascular densities(MVDs)of group A,group B,and group C were 44.5%±6.6%,47.2%±7.4%,48.4%±5.3%,respectively.The difference in deep MVD among the 3 groups was statistically significant(F=3.449,P=0.035).In the pairwise comparison,group A was lower than group C,and the difference was statistically significant(P=0.015).There was no statistically significant difference between group A and group B or between group B and group C.There was no significant difference in the area of superficial MVD and macular avascular zone(FAZ)among the three groups.The retinal light sensitivity(RS)of group A,group B,and group C was 27.7±2.2 dB,28.0±1.8 dB,and 29.2±1.0 dB,respectively.The difference in RS among the three groups was statistically significant(F=5.816,P=0.004).In the pairwise comparison,groups A and B were lower than group C,and th
