摘 要:目的:探讨膜引导组织再生技术(GTR)联合自体骨移植在牙周骨下缺损的临床效果。方法:将2016年1月至2019年5月在漯河医学高等专科学校第二附属医院60例(95颗患牙)牙周骨下缺损患者随机分为对照组(30例,46个位点)和观察组(30例,49个位点),均进行常规翻瓣术治疗,对照组给予自体骨移植治疗,观察组给予GTR联合自体骨移植治疗,对比两组患者术前及术后24周的术区同位点牙槽骨密度与骨缺损高度、牙周组织指标、龈沟液中成骨细胞转化生长因子-β1(TGF-β1)和骨形态发生蛋白-1(BMP-1)水平,评估术前及术后6个月的牙龈美容形态评分。结果:术后24周,两组患者的同位点牙槽骨密度较术前增加(P<0.05),骨缺损高度较术前降低(P<0.05),但两组间术后24周两项指标变化比较,差异未见统计学意义(P>0.05);术后24周,两组患者的牙周临床附着丧失(CAL)、牙周袋深度(PD)较术前缩短(P<0.05),出血指数(BI)较术前降低(P<0.05),观察组GAL值短于对照组(P<0.05);术后24周,两组患者龈沟液中TGF-β、BMP-1水平较术前提高(P<0.05),观察组高于对照组(P<0.05);术后6个月,两组的牙龈美容形态评分较术前提高(P<0.05),观察组高于对照组(P<0.05)。结论:GTR联合自体骨移植能有效促进牙周骨下缺损的修复和新生,患者术区牙龈美容形态良好。Objective To investigate the clinical effects of membraneguided tissue regeneration(GTR)combined with autogenous bone transplantation on subperiodontal bone defects.Methods Sixty patients(95 affected teeth)with subperiodontal bone defects in the second Affiliated Hospital of Louhe Medical College from January 2016 to may 2019,amd they were randomly divided into control group(30 cases,46 loci)and observation group(30 cases,49 loci),and they were treated with conventional flap surgery,and control group was given autologous bone transplantation,and observation group was given GTR combined with autologous bone transplantation.The alveolar bone density and bone defect height at the same locus of surgical area,periodontal tissue indexes and levels of transforming growth factor B(TGF-β)and bone morphogenetic protein-1(BMP-1)in gingival crevicular fluid were compared between the two groups before surgery and 24 weeks after surgery.The gingival cosmetic morphology score were evaluated before surgery and 6 months after surgery.Results Twenty-four weeks after surgery,the alveolar bone mineral density at the same locus in the two groups was increased significantly compared with that before surgery(P<0.05),and the bone defectheight was significantly decreased compared with that before surgery(P<0.05),and there were no statistically significant differencesin the changes of indexes at 24 weeks after surgery(P>0.05).Twenty-four weeks after surgery,the periodontal clinical attachment loss(CAL)and periodontal pocket depth(PD)in the two groups were significantly shortenedcompared with those before surgery(P<0.05),and the bleeding index(BI)was significantly decreased compared with that before surgery(P<0.05),and the GAL value in observation group was significantly shorter than that in control group(P<0.05).At 24 w after surgery,the levels of TGF-β and BMP-1 in gingival crevicular fluid in the two groups were improved compared with those in control group(P<0.05),and the levels in observation group were significantly higher than tho
