摘　　要：目的探讨鼻腔鼻窦恶性黑色素瘤(SNM)的临床特征、诊断与治疗,并分析影响其预后的因素。方法选择2011年8月至2019年8月郑州大学第一附属医院收治的SNM患者42例,回顾性分析并总结其临床特点、疗效及预后。结果42例SNM患者中,男28例,女14例;年龄56~82岁,平均年龄63.1岁。19例患者行手术+放疗治疗,14例患者行手术+放疗+靶向治疗,9例行手术+放疗+化疗治疗。随访时间5~69个月,中位随访时间40个月,5年总生存率为7.1%(3/42)。结论SNM发病率低,侵袭性强,早期诊断及根治性手术切除仍然是目前SNM患者预后的重要影响因素,先进的辅助和新辅助靶向化疗可能是未来治疗SNM的主要方法。Objective To investigate the clinical characteristics,diagnosis and treatment of sinonasal malignant melanoma(SNM),and to analyze the factors influencing the prognosis.Methods The clinical data of 42 patients with SNM admitted to the First Affiliated Hospital of Zhengzhou University from August 2011 to August 2019 were analyzed retrospectively.The clinical characteristics,curative effect and prognosis were analyzed and summarized.Results Among the 42 SNM patients,28 cases were males and 14 cases were females,aged 56-82 years,and the average age was 63.1 years old.Nineteen patients received surgery and radiotherapy,and 14 patients received surgery,radiotherapy and targeted therapy,while 9 patients received surgery,radiotherapy and chemotherapy.The follow-up time ranged from 5 to 69 months,and the median follow-up time was 40 months.The 5-year overall survival rate of sinonasal malignant melanoma was 7.1%(3/42).Conclusions SNM is an invasive disease with low prevalence.Early diagnosis and radical surgical resection are still important factors affecting the prognoses of patients with sinonasal malignant melanoma.Advanced adjuvant and neoadjuvant targeted chemotherapy may be the main treatment methods for SNM in the future.
