摘  要:目的探讨人类白细胞抗原-E(HLA-E)、人类白细胞抗原-G(HLA-G)在早期宫颈癌及癌前病变中的表达及其与人乳头瘤病毒(HPV)感染的相关性研究。方法取80例早期宫颈癌患者的宫颈癌组织,120例宫颈上皮内瘤变(CIN)患者的CIN组织,150例慢性宫颈炎患者的正常宫颈组织。免疫组化法检测HLA-E、HLA-G蛋白的阳性表达率和HPV负荷量,分析其与宫颈癌患者临床特征的相关性。将siE6和阴性对照(NC)转染至人宫颈癌细胞株HeLa,得到HeLa-siE6及HeLa-NC细胞株,以未处理的HeLa细胞作为空白对照。蛋白质印迹(Western blot)法检测不同HPV感染状态和沉默E6基因的表达后HLA-E、HLA-G蛋白的相对表达量。结果随病变程度的加重,HLA-E、HLA-G蛋白的阳性表达率和HPV阳性率均逐渐升高,其中宫颈癌组织中HLA-E、HLA-G蛋白的阳性表达率和HPV阳性率均高于CIN组织和慢性宫颈炎组织,CIN组织中HLA-E、HLA-G蛋白的阳性表达率和HPV阳性率均高于慢性宫颈炎组织,差异均有统计学意义(P﹤0.05)。HPV病毒负荷量也随着病变程度的增加而升高。相关性分析显示,HLA-E和HLA-G蛋白的表达与分化程度、CIN分级、TNM分期和HPV感染均呈正相关,与年龄、肿瘤直径、淋巴结转移情况无相关性,且HLA-E的表达与HLA-G呈正相关关系(P﹤0.05)。HeLa-siE6细胞中E6 mRNA的相对表达量,均明显低于HeLa细胞和HeLa-NC细胞,差异均有统计学意义(P﹤0.01)。人宫颈癌HeLa-siE6细胞中HLA-E、HLA-G蛋白的相对表达量,均明显低于HeLa和HeLa-NC细胞,差异均有统计学意义(P﹤0.01)。结论HLA-G、HLA-E的表达水平与HPV感染均随着宫颈病变程度的增加而升高,且相互之间呈正相关关系,HLA-G、HLA-E与HPV感染类型无关,可能共同参与了宫颈癌的进展。Objective To investigate the expression of human leukocyte antigen-E(HLA-E)and human leukocyte antigen-G(HLA-G)in early-stage cervical cancer and precancerous lesions and their correlation with human papillomavirus(HPV)infection.Method Cervical tissue samples were collected from 80 patients with early cervical cancer,120 patients with cervical intraepithelial neoplasia(CIN)and 150 patients with chronic cervicitis.The expression levels of HLAE and HLA-G and the viral loads of HPV were detected by immunohistochemistry assay and their correlation with clinical features of patients with cervical patients were analyzed.The siE6 and negative control(NC)were transfected into the human cervical cancer cell line HeLa to obtain HeLa-siE6 and HeLa-NC cell lines,the untreated HeLa cells were used as the blank control.Western blot method was used to detect the relative expression of HLA-E and HLA-G protein after different HPV infection status and the expression of silenced E6 gene.Result With the aggravation of the disease,the positive expression rate of HLA-E and HLA-G,together with the positive rate of HPV infection,gradually increased.Among them,the positive expression rate of HLA-E and HLA-G in cervical cancer tissues and the positive rate of HPV infection were all higher than in neither CIN tissues nor chronic cervicitis tissues,and the positive expression rate of HLA-E and HLA-G protein and HPV positive rate in CIN tissues were higher than those in chronic cervicitis tissues,and all the differences were statistically significant(P<0.05).The viral loads of HPV were also uplifting with the progression of the disease.Correlation analysis showed that HLA-E and HLA-G were positively correlated with the degree of differentiation,CIN grade,TNM staging,and HPV infection,but were not correlated with factors like age,diameter of tumor,lymph node metastasis.Besides,the expression of HLA-E and HLA-G indicated a positive correlation(P<0.05).The expression levels of E6 mRNA and relative expression of HLA-E and HLA-G were lower in HeL
