分析子宫肌瘤患者HIFU术消融率、肌瘤缩小率与复发情况的关系

摘　　要：目的分析子宫肌瘤患者高强度聚焦超声(HIFU)术后消融率、肌瘤缩小率与复发情况的关系。方法回顾性分析2016年1月至8月新疆生产建设兵团第四师医院行HIFU治疗的围绝经期子宫肌瘤患者100例,分析HIFU治疗结果,比较不同类型子宫肌瘤消融率,治疗后肌瘤体积以及治疗1年后随访情况。结果100例患者肌瘤的平均消融率为(80±19)%;磁共振T2WI低信号肌瘤的消融率明显高于高信号肌瘤;与治疗前相比,治疗后3个月、6个月、9个月、1年肌瘤体积缩小率分别为46.73%、64.34%、79.21%、83.10%,逐渐升高(P<0.05);治疗后1年随访,其中90例患者肌瘤体积均小于治疗前,肌瘤平均消融率为(88.86±11.67)%;10例患者肌瘤体积略有增大,肌瘤平均消融率为(71.34±12.26)%;总体局部复发率为13.00%;多发性肌瘤及术前血供较丰富的肌瘤在HIFU治疗后易复发。结论HIFU治疗子宫肌瘤的消融率与复发相关,消融率越高,术后吸收率越高,复发概率越低;反之,消融率越低,HIFU后肌瘤缩小率低,复发概率大。Objective To analyze the relationship between ablation rate,fibroid shrinkage rate and recurrence after HIFU in patients with uterine fibroids.Methods From January to August 2016,100 patients with perimenopausal uterine fibroids underwent high-intensity focused ultrasound(HIFU)treatment in Xinjiang Construction Corps Fourth Division Hospital were analyzed.The results of HIFU treatment were analyzed and the ablation rates of different types of uterine fibroids were compared.The volume of fibroids after treatment and follow-up after 1 year of treatment.Results The average ablation rate of fibroids in 100 patients was(80±19)%.The ablation rate of MRI T2WI low-signal fibroids was significantly higher than that of high-signal fibroids.The reduction rates of fibroid volume at months,9 months,and 1 year were 46.73%,64.34%,79.21%,and 83.10%,respectively.After 1 year of follow-up,90 patients had fibroid volume smaller than that before treatment,and 10 patients The fibroid volume increased slightly,and the overall local recurrence rate was 10%.The average ablation rate of fibroids in 10 patients with enlarged fibroids was(71.34±12.26)%,and the average ablation rate of fibroids in the remaining 90 patients with reduced fibroids volume was(88.86±11.67)%;the fibroid volume increased slightly in 10 patients.The average ablation rate of fibroids was(71.34±12.26)%;the overall local recurrence rate was 13.00%;multiple fibroids and fibroids with abundant blood supply before surgery tend to relapse after HIFU treatment.Conclusion The ablation rate of HIFU for uterine fibroids is related to recurrence.The higher the ablation rate,the higher the postoperative absorption rate,and the lower the probability of recurrence.Conversely,the lower the ablation rate,the lower the rate of fibroid shrinkage after HIFU and the greater the probability of recurrence.
