摘 要:目的对比43例儿童人工耳蜗植入(Cochlear Implant,CI)前后不同时间、不同辅听模式下声源定位能力的变化情况。方法通过对比43例CI患儿不同辅听模式以及听力正常儿童的角度辨别阈值(Minimum Audible An⁃gle,MAA),初步判断儿童不同听力模式下声源定位能力的差别。结果CI术后6个月CI+助听器(Hearing aid,HA)模式下与CI植入前MAA角度变化不大,术后12个月时CI+HA模式下MAA角度较术前及术后6个月明显减小;CI术后12个月,双侧CI模式MAA角度小于CI+HA双模式佩戴及单侧人工耳蜗佩戴模式;其MAA角度与正常儿童单耳听力状态水平相当,与正常听力儿童MAA角度仍有差距。结论CI在改善耳聋患儿听力的同时对患儿声源定位能力在一定程度上有所提高,CI术后患儿的声源定位能力随着时间的延长可进一步改善,双侧CI术后声源定位能力可能优于其他辅助听力佩戴模式,但较正常儿童仍有差距。Objective To compare changes of sound localization ability in different time and auxiliary-hearingmodes before and after Cochlear Implant(CI)in children.Methods Source location capabilities under different hearing modes were judged by comparing the different auxiliary hearing patterns and the Minimum Audible Angle(MAA)in children at the Department of Otolaryngology Head and Neck Surgery of Beijing Chaoyang Hospital and Peking Union Medical College Hospital.Results The angle of MAA in CI+Hearing-Aid mode was not significantly different than before implantation at 6 months after CI implantation,but was significantly smaller than before implantation after 12 months.After 1 year of implantation,MAA angles under bilateral CI wearing mode were less than the CI+hearing aid and single side cochlear modes and could reach the single ear level in normal children.Conclusion In addition to hearing ability,CI can also improve sound source localization ability with the extension of time in children with deafness,especially with bilateral CI,although there is still a gap from children with normal hearing.
