血清AFP、CEA、CA199、CA724联合检测对消化道恶性肿瘤的诊断价值

摘　　要：目的探究血清AFP、CEA、CA199、CA724联合检测对消化道恶性肿瘤的诊断价值。方法选取2017年1月~2019年1月我院收治的消化道恶性肿瘤患者160例设为研究组,其中肝癌35例,胰腺癌42例,胃癌37例,大肠癌46例;另选取同期健康体检者45名设为参照组;比较两组血清AFP、CEA、CA199、CA724水平,分析血清AFP、CEA、CA199、CA724单独检测及联合检测不同消化道恶性肿瘤的阳性率、灵敏度及特异度。结果研究组中肝癌、胰腺癌、胃癌、大肠癌血清AFP、CEA、CA199、CA724水平高于参照组,差异有统计学意义(P<0.05);AFP+CEA+CA199+CA724联合检测肝癌、胰腺癌、胃癌及大肠癌的阳性率、灵敏度及特异度高于任一项单独检测值,差异有统计学意义(P<0.05)。结论血清AFP、CEA、CA联合检测可作为判断消化道恶性肿瘤的重要指标,且联合检测可提高诊断的阳性率、灵敏度及特异度。Objective To explore the diagnostic value of combined detection of serum AFP,CEA,CA199 and CA724 for malignant tumors of the digestive tract.Methods A total of 160 patients with gastrointestinal malignancies admitted to our hospital from January 2017 to January 2019 were selected as the study group,including 35 liver cancers,42 pancreatic cancers,37 gastric cancers,and 46 colorectal cancers;the other healthy patients in the same period were selected.45 physical examination subjects were set as the reference group;the serum levels of AFP,CEA,CA199,and CA724 were compared between the two groups,and the positive rate,sensitivity,and specificity of serum AFP,CEA,CA199,and CA724 were analyzed separately and combined with different gastrointestinal malignancies.Results The serum levels of AFP,CEA,CA199,and CA724 in liver cancers,pancreatic cancers,gastric cancers,and colorectal cancers of the study group were higher than those in the reference group,the difference was statistically significant(P<0.05);the positive rate and sensitivity of AFP+CEA+CA199+CA724 combined detection of liver cancer,pancreatic cancer,gastric cancer and colorectal cancer and the specificity was higher than any of the individual detection values,the difference was statistically significant(P<0.05).Conclusion The combined detection of serum AFP,CEA,and CA can be used as an important indicator for judging malignant tumors of the digestive tract,and the combined detection can improve the positive rate,sensitivity and specificity of diagnosis.
