白内障合并中低度数角膜散光患者植入散光型人工晶体的效果及主观视觉质量分析

摘　　要：目的对比研究伴有中低度数角膜散光的白内障患者行白内障超声乳化术并植入散光矫正型晶体后的临床效果及主观视觉质量。方法收集确诊为规则性角膜散光(1.0~2.0D)的年龄相关性白内障患者122眼,行白内障超声乳化术,分别植入散光矫正型晶体及非球面晶体。观察并比较术前及术后1周、1个月、6个月、1年的裸眼远视力(UCVA)、最佳矫正远视力(BCVA),术前及术后3个月、6个月、1年的显然验光,术后1 d、1周、1个月、3个月、1年的散光晶体轴位变化,术后3个月的主观视觉质量评分。结果2组患者术后各时间点的UCVA及BCVA均较术前提高,差异有统计学意义(P<0.05),散光晶体组术后1年的UCVA较其他时间点下降,差异有统计学意义(P<0.05),2组患者术后各时间点显然验光中散光均较术前降低,差异有统计学意义(P<0.05);裂隙灯下观察,3个月散光晶体旋转基本稳定,术后3个月散光晶体组的远视力、白天视力、重影、术后脱镜率(远)以及整体术后主观视觉质量评分均好于非球面晶体组(P<0.05)。结论散光矫正型晶体具有稳定的散光矫正效果,更好的主观视觉质量。Objective To investigate the clinical effects and subjective visual quality of patients with cataract complicated by moderate or low degree corneal astigmatism undergoing cataract phacoemulsification and implanted astigmatism.Methods A total of 122 eyes of patients with age related cataract who were diagnosed as regular corneal astigmatism(1.0D^2.0D)in our hospital were enrolled in the study.The patients were treated by cataract phacoemulsification,and were implanted with astigmatism intraocular lens and aspheric intraocular lens.The uncorrected visual acuity(UCVA),best corrected visual acuity(BCVA)before operation and at 3m,6m,1y after operation were observed.Moreover the changes of astigmatism axis lentis;axis of lens at 1d,1w,1 m,3m,1y after operation,and the subjective visual quality score at 3m after surgery were observed.Results After operation the UCVA and BCVA at each time point in both group were significantly improved,as compared with those before operation(P<0.05).The UCVA at 1y after operation in astigmatism intraocular lens group was significantly decreased,as compared with that in the other time points(P<0.05).The astigmatism in manifestrefraction in both groups at each time point after operation was significantly decreased,as compared with that before operation(P<0.05).Under the slit lamp,the crystal rotation of the astigmatism crystal was basically stable at 3m after operation.Moreover the subjective visual quality scores,distant vision,daytime vision,double vision,postoperative lens removal rate(far)and overall postoperative visual quality scores in astigmatism crystal group were superior to those in aspherical crystal group(P<0.05).Conclusion Astigmatism corrected crystal has better astigmatism correction effect,better subjective visual quality.
