摘　　要：目的探讨即刻早期基因Cyr61在宫颈癌中的表达及与高危型人乳头瘤病毒(HPV)感染的关系.方法收集183例宫颈病变患者,根据术后病理检查结果分为良性病变组、宫颈上皮内瘤变组、宫颈癌组,留存宫颈病变组织,免疫组化检测Cyr61表达水平,并分析Cyr61阳性率与患者病理特征的关系.采用RT-PCR及Western blot法检测人宫颈癌细胞株C33a、Caski mRNA和蛋白表达,并使用HPV16 E6特异性抑制序列siRNA转染Caski细胞株72 h,检测Cyr61 mRNA和蛋白表达.结果宫颈癌组Cyr61阳性率明显低于良性病变组和宫颈上皮内瘤变组,宫颈上皮内瘤变组Cyr61阳性率明显低于良性病变组,差异具有统计学意义(P<0.01).不同分化程度、浸润深度、淋巴结转移情况的宫颈癌患者中Cyr61阳性率差异具有统计学意义(P<0.05),其中分化程度G 1~G 2、无宫旁浸润、有淋巴结转移的宫颈癌患者Cyr61阳性率偏高.宫颈癌细胞株C33a的Cyr61 mRNA和蛋白表达水平明显高于宫颈癌细胞株Caski,差异具有统计学意义(P<0.01).siRNA组宫颈Caski细胞中Cyr61mRNA和蛋白表达水平明显高于空白对照组和阴性对照组,差异具有统计学意义(P<0.05).结论Cyr61在宫颈癌组织中低表达,且与病变程度呈负相关;HPV16可能通过E6蛋白相关信号通路下调Cyr61的表达,促进宫颈癌的发生与发展.Objective To investigate the expression of immediate early gene Cyr61 in cervical cancer and its relationship with high-risk human papillomavirus(HPV)infection.Method 183 patients with cervical lesions who underwent surgical treatment were collected.Paraffin mass of cervical lesions was retained.Cyr61 expression level was detected by immunohistochemistry,and the relationship between Cyr61 positive rate and pathological characteristics was analyzed.RT-PCR and Western blot were used to detect the expression of Caski and C33a in human cervical cancer cell lines,and the expression of Cyr61 was detected 72 hours after transfection with siRNA of HPV16 E6 specific inhibitory sequence.Results The positive rate of Cyr61 in cervical cancer group was significantly lower than that in benign lesion group and cervical intraepithelial neoplasia group.The positive rate of Cyr61 in cervical intraepithelial neoplasia group was significantly lower than that in benign lesion group(P<0.01).The positive rate of Cyr61 in cervical cancer patients with different degrees of differentiation,depth of invasion and lymph node metastasis was significantly different(P<0.05);among them,the positive rate of Cyr61 in cervical cancer patients with degree of differentiation G 1~G 2,no parauterine invasion and lymph node metastasis was higher.The expression level of Cyr61 in cervical cancer cell line C33a was significantly higher than that in cervical cancer cell line Caski(P<0.01).The expression of Cyr61 in CaSki cells of siRNA group was significantly higher than that of blank control group and negative control group(P<0.05).Conclusion Cyr61 expression is low in cervical cancer tissues and negatively correlated with the degree of lesion.HPV16 may down-regulate Cyr61 expression through E6 protein-related signaling pathway to promote the occurrence and development of cervical cancer.
