摘  要:目的探讨认知训练改善缺血性脑卒中后血管性认知功能障碍的临床效果。方法选择2016年7月~2019年3月我院诊治的78例缺血性脑卒中后认知功能障碍患者,随机分为观察组和对照组。两组予口服奥拉西坦胶囊0.8 g/次,4次/d,连用8周。对照组患者予常规干预措施,观察组患者在对照组基础上加用认知训练,两组均干预8周。观察两组干预前后认知功能指标的变化,并评估其临床效果。结果干预8周后,两组MMSE评分、WMS评分和画钟试验评分较前明显上升(P<0.05或P<0.01),且观察组上升幅度较对照组更显著(P<0.05);同时观察组患者认知功能改善总有效率较对照组更佳(χ2=4.13,P<0.05)。结论认知训练改善缺血性脑卒中后血管性认知功能障碍的临床效果肯定,明显延缓认知功能障碍进展,改善其认知功能。Objective To investigate the clinical effect of cognitive training on improving vascular cognitive dysfunction after ischemic stroke.Methods A total of 78 patients with post-ischemic stroke cognitive dysfunction treated in our hospital from July 2016 to March 2019 were enrolled and randomly divided into the observation group and the control group.Oxiracetam capsules were administered orally 0.8 g/time and 4 times/d for 8 weeks in both groups.Patients in the control group were given routine intervention and patients in the observation group were given cognitive training on the basis of the control group.Both groups were intervened for 8 weeks.The changes of cognitive function indicators in the two groups before and after the intervention were observed and the clinical effects were evaluated.Results After 8 weeks of intervention,the MMSE scores,WMS scores and clock drawing task scores of the two groups were significantly increased compared to those before the intervention(P<0.05 or P<0.01),and the increase extent of the observation group was more significant than that of the control group(P<0.05).The total effective rate of cognitive function improvement in the group was better than that in the control group(χ2=4.13,P<0.05).Conclusion Cognitive training has a positive clinical effect on improving vascular cognitive dysfunction after ischemic stroke,and can significantly delay the progression of cognitive dysfunction,and improve the cognitive function.
