腹腔镜超低位直肠癌保肛术治疗超低位直肠癌的效果分析

摘　　要：目的:分析用腹腔镜超低位直肠癌保肛术治疗超低位直肠癌的临床疗效。方法:从在桂林市第二人民医院接受治疗的超低位直肠癌患者中抽选60例患者作为研究对象。这些患者的收治时间均为2014年4月至2016年4月。将其随机分为Open组和Laparoscopic组。为Open组30例患者采用常规的开腹超低位直肠癌根治术进行治疗,为Laparoscopic组30例患者采用腹腔镜超低位直肠癌保肛术进行治疗。然后比较两组患者的手术指标、保肛率、术后1年、2年、3年的存活率及术后并发症的发生情况。结果:1)Laparoscopic组患者的保肛率高于Open组患者,P<0.05。两组患者术后1年、2年、3年的存活率相比,P>0.05。2)Laparoscopic组患者术中的出血量少于Open组患者,其术中清扫淋巴结的数目多于Open组患者,其术后疼痛症状消失的时间短于Open组患者,P<0.05。结论:用腹腔镜超低位直肠癌保肛术治疗超低位直肠癌具有创伤小、保肛率高、患者术后恢复快等优点。Objective:To analyze the clinical efficacy of laparoscopy in the treatment of ultra-low rectal cancer with anus preserving.Methods:60 patients with ultra-low rectal cancer were selected from the second People’s Hospital of Guilin.The patients were admitted between April 2014 and April 2016.And it was randomly divided into Open group and Planning group.In addition,30 patients in the Open group were treated with conventional radical surgery for ultra-low rectal cancer,and 30 patients in the Open group were treated with Laparoscopic ultra-low rectal cancer and anal conservation.Then the operative indexes,the rate of preserving anus,the survival rate at 1,2 and 3 years after operation,and the incidence of postoperative complications were compared between the two groups.Results:1)The anal conservation rate of patients in the Open group was higher than that of patients in the Open group,P<0.05.The survival rate at 1,2 and 3 years after operation wa s compared between the two groups,and P>0.05.2)Compared with patients in the Open group,the amount of blood loss in the surgery was less than that of patients in the Open group,and the number of intraoperative lymph nodes dissected was higher than that of patients in the Open group,and the duration of postoperative pain symptoms was shorter than that of patients in the Open group,P<0.05.Conclusion:Laparoscopy for the treatment of ultra-low rectal cancer with anus preserving has the advantages of less trauma,higher anus preserving rate and faster postoperative recovery.
