摘　　要：目的观察结构和心理授权联合模式在老年脑卒中病人中的应用效果。方法收集2017年2月至2019年2月太和县人民医院的老年脑卒中病人86例,按授权理念管理方法分成2组,其中对照组45例采用常规健康教育方法,联合授权组41例采用结构和心理授权联合教育模式进行干预。观察2组病人出院后6个月神经功能、康复锻炼依从性、日常生活活动能力、自我效能感、授权能力、生活质量及抑郁和焦虑状态的变化。结果出院后6个月,与对照组相比,联合授权组的NIHSS、焦虑自评量表(SAS)及抑郁自评量表(SDS)评分均降低(P<0.05),而授权能力评分、脑卒中自我效能感量表(SSEQ)评分、Barthel指数、康复锻炼依从性评分及生活质量量表(SF-36)评分均升高(P<0.05)。结论对老年脑卒中病人采用结构和心理授权联合模式进行管理,可有效促进神经功能恢复,改善生活质量,提高病人的授权能力和自我管理水平,并有效缓解病人抑郁和焦虑情绪。Objective To observe the effect of the combined model of structure and psychological empowerment in the elderly patients with stroke.Methods Eighty-six elderly patients with stroke hospitalized in Taihe County People's Hospital from February 2017 to February 2019 were enrolled.Patients were divided into two groups according to the model of education.In the control group,45 patients accepted routine health education.In the joint empowerment management group,41 cases were intervened by a joint education model of structure and psychological empowerment.Six months after discharge,neurological function,self-efficacy,rehabilitation exercise compliance,activities of daily living,quality of life,empowerment ability and changes in scores of depression and anxiety were observed.Results Compared to control group,the scores of National Institutes of Health Stroke Scale(NIHSS),Self-rating Anxiety Scale(SAS)and Self-rating Depression Scale(SDS)were lower in the joint empowerment management group(P<0.05),and the scores of empowerment ability,stroke self-efficacy scale(SSEQ),Barthel index,rehabilitation exercise compliance and quality of life(CSF-36)were all increased(P<0.05)six months after discharge.Conclusions The management of joint model of structural and psychological empowerment in the elderly patients with stroke can effectively promote the recovery of neurological function,improve the quality of life,increase the empowerment ability and self-management level,and effectively alleviate the depression and anxiety.
