体检超声在肾血管平滑肌脂肪瘤与无症状肾癌鉴别诊断中的应用价值

摘　　要：目的探讨体检超声在肾血管平滑肌脂肪瘤与无症状肾癌鉴别诊断中的应用价值。方法回顾性分析2015年4月至2019年12月在某院健康检查并经病理证实的45例肾脏肿瘤资料,根据病理性质分为肾癌组和肾错构瘤组。回顾分析病灶的大小、形态、位置、边缘、边界、内部回声等情况,彩色及频谱多普勒超声观察肿瘤血供情况,同时注意观察肾静脉及下腔静脉内有无栓子及肾周淋巴结的转移。结果两组间在年龄、肿块大小、形态、生长位置、边缘边界方面差异无统计学意义(P>0.05),而在肿块回声强度、内部结构以及CDFI方面差异有统计学意义(P<0.05)。肾癌组主要表现为回声强度较同切面肾窦层切面高,内部结构多均匀,血流信号少见。结论细致分析体检超声肾脏肿瘤的二维和多普勒超声特点,有助于肾癌和肾错构瘤的术前鉴别诊断。Objective To explore the value of ultrasonography in the differential diagnosis of renal angioleiomyolipoma and asymptomatic renal carcinoma.Methods Retrospective analysis was made on the data of 45 cases of renal tumors confirmed by pathology in a hospital from April 2015 to December 2019,which were divided into renal cancer group and renal hamartoma group according to their pathological characteristics.The size,shape,location,edge,boundary and internal echo of the lesion were retrospectively analyzed.The blood supply of the tumor was observed by color and spectral Doppler ultrasound,and the presence of embolus in the renal vein and inferior vena cava and metastasis of perirenal lymph nodes were also observed.Results There were no statistically significant differences in age,mass size,shape,growth location and edge boundary between the two groups(P>0.05),while there were statistically significant differences in echo intensity,internal structure and CDFI between the two groups(P<0.05).In the renal cancer group,the echo intensity was higher than that in the same section of renal sinus layer,the internal structure was more uniform,and the blood flow signal was rare.Conclusion Detailed analysis of the two-dimensional and Doppler ultrasonographic characteristics of renal tumors in physical examination is helpful for preoperative differential diagnosis of renal cancer and renal hamartoma.
