遗忘型轻度认知功能障碍人群听觉语言工作记忆和执行功能特点

摘　　要：目的研究遗忘型轻度认知功能障碍(MCI)患者听觉语言工作记忆和执行功能特点。方法选取33例遗忘型MCI患者作为研究组,另选取同期33例无脑内病变的健康老年人作为常规组。两组均接受神经心理学测试、词语流畅性测试、数字广度测验。对比两组的神经心理学测验结果以及词语流畅性测试评分、数字广度测验评分。结果研究组简易智力状态检查量表(MMSE)评分(24.6±0.7)分低于常规组的(29.8±0.6)分,临床痴呆评定量表(CDR)评分(0.5±0.1)分高于常规组的0分,差异具有统计学意义(P<0.05);但两组汉密尔顿抑郁量表(HAMD)评分以及日常生活活动能力(ADL)评分对比,差异无统计学意义(P>0.05)。研究组词语流畅性测试评分(23.5±2.5)分、逆序数字广度测验评分(1.4±0.3)分均明显低于常规组的(32.6±3.8)、(3.7±0.6)分,差异具有统计学意义(P<0.05);但两组顺序数字广度测验评分对比,差异无统计学意义(P>0.05)。结论遗忘型MCI患者的听觉语言、工作记忆、执行功能明显受损。Objective To study the characteristics of auditory-verbal working memory and executive function in patients with amnestic mild cognitive impairment(MCI).Methods There were 33 patients with amnestic mild cognitive impairment selected as the research group,and another 33 healthy old people without intracerebral lesions in the same period as the conventional group.Both groups received neuropsychological tests,word fluency test and digit span test.The neuropsychological test results,word fluency test scores and digit span test scores were compared between the two groups.Results The mini mental state examination(MMSE)score(24.6±0.7)points of the research group was lower than that of the conventional group(29.8±0.6)points,clinical dementia rating(CDR)score(0.5±0.1)points was higher than that of the conventional group,and the difference was statistically significant(P<0.05).But there was no statistically significant difference in Hamilton depression rating scale(HAMD)and activities of daily living(ADL)score between the two groups(P>0.05).The word fluency test scores(23.5±2.5)points and reverse digit span test score(1.4±0.3)points of the research group were obviously lower than those of the conventional group(32.6±3.8)and(3.7±0.6)points,and the difference was statistically significant(P<0.05).There was no statistically significant difference in sequential digit span test score between the two groups(P>0.05).Conclusion The auditory-verbal working memory and executive function of amnestic MCI patients are significantly impaired.
