激素受体阳性乳腺癌长期内分泌治疗不良反应的观察

摘　　要：目的探讨激素受体阳性乳腺癌长期内分泌治疗的不良反应。方法对接受1年内分泌治疗发生不良反应的175例激素受体阳性乳腺癌患者的临床资料进行回顾性分析,统计分析患者的不良反应类型,并分析不良反应患者的分布情况。结果175例发生不良反应的患者中,潮热43例(24.57%),乏力32例(18.29%),关节痛27例(15.43%),恶心21例(12.00%),脱发15例(8.57%),皮疹12例(6.86%),食欲不振或增加10例(5.71%),抑郁8例(4.57%),血栓7例(4.00%)。175例发生不良反应的患者中,其中<55岁患者72例,≥55岁患者103例;未绝经患者56例,已绝经患者119例;体力状况(PS)评分0分患者32例,1分患者65例,2分患者78例;Ki67<14%患者60例,Ki67≥14%患者115例;雌激素受体(ER)状态+患者37例,++患者62例,+++患者76例;有内脏转移患者78例,无内脏转移患者97例。年龄≥55岁、已绝经、PS评分2分、Ki67≥14%、ER状态+++、无内脏转移占比更高。结论在对激素受体阳性乳腺癌患者长期内分泌治疗时,会发生较多的不良反应,需要加强临床观察,重视内分泌治疗过程中不良反应的全程管理。Objective To discuss the adverse effects of long-term endocrine therapy on hormone receptor positive breast cancer.Methods The clinical data of 175 patients with hormone receptor positive breast cancer who had adverse reactions in endocrine therapy for 1 year were analyzed retrospectively.The types of adverse reactions were statistically analyzed,and the distribution of adverse reactions was analyzed.Results Of the 175 patients with adverse reactions,there were 43 cases(24.57%)of hot flashes,32 cases(18.29%)of fatigue,27 cases of(15.43%)arthralgia,21 cases(12.00%)of nausea,15 cases(8.57%)of alopecia,12 cases(6.86%)of rash,10 cases(5.71%)of anorexia or increase,8 cases(4.57%)of depression,7 cases(4.00%)of thrombosis.Among the 175 patients with adverse reactions,72 cases were<55 years old,103 cases were≥55 years old;56 cases were non-menopausal,and 119 cases were menopausal;physical status(PS)score of 32 patients was 0 point,65 patients were 1 point and 78 patients were 2 points;60 patients with Ki67<14%,115 patients with Ki67≥14%;37 patients with estrogen receptor(ER)status+,62 patients with ER status++,76 patients with ER status+++;78 patients with visceral metastasis,97 patients with none visceral metastasis.The proportion of age≥55 years old,menopausal,PS score 2 points,Ki67≥14%,ER status+++and without visceral metastasis was higher.Conclusion There are many adverse reactions in the long-term endocrine therapy for hormone receptorpositive breast cancer patients,so it is necessary to strengthen clinical observation and pay attention to the whole management of adverse reactions in the process of endocrine therapy.
