微导管辅助360°小梁切开术治疗青少年开角型青光眼的疗效观察

摘　　要：目的评估微导管辅助360°小梁切开术治疗青少年开角型青光眼(JOAG)的临床效果。方法选取60例(86眼)青少年开角型青光眼患者作为研究对象,所有患者均采用微导管辅助360°小梁切开术进行治疗。观察患眼成功率及并发症发生情况,比较手术前后的眼内压。结果患者手术后眼内压为(14.4±4.3)mm Hg(1mm Hg=0.133 kPa),低于手术前的(30.4±7.1)mm Hg,差异有统计学意义(t=17.876,P<0.05)。手术成功率为90.7%(78/86),随访期平均为15个月。手术后有5眼出现暂时性眼前房积血。有2例患者之前接受过激光小梁形成术,手术时间4个月~4年,其未发生并发症,手术成功。结论对青少年开角型青光眼患者进行微导管辅助360°小梁切开术临床疗效显著,其手术成功率更高、并发症较少,值得临床推广。Objective To evaluated the clinical effect of 360-degree trabeculotomy assisted by microcatheter in the treatment of juvenile open-angle glaucoma(JOAG).Methods There were 60 cases(86 involved eyes)of juvenile open-angle glaucoma as study subjects,and all were treated by 360-degree trabeculotomy assisted by microcatheter.The success rate of the affected eye and the occurrence of complications was observed,and the intraocular pressure before and after the surgery was compared.Results The intraocular pressure was(14.4±4.3)mm Hg(1 mm Hg=0.133 kPa)after the surgery,which was lower than that before the surgery(30.4±7.1)mm Hg,and the difference was statistically significant(t=17.876,P<0.05).The success rate of surgery was 90.7%(78/86),and the average follow-up period was 15 months.There were 5 eyes with temporary hyphema after the surgery.2 patients had undergone laser trabeculoplasty before,and the surgery time was 4 months-4 years.No complications occurred and the surgery was successful.Conclusion 360-degree trabeculotomy assisted by microcatheter shows remarkable clinical efficacy for juvenile open-angle glaucoma,with a higher surgical success rate and fewer complications,which is worthy of clinical promotion.
