摘　　要：目的观察高血压脑出血患者外周血补体C3、C4动态变化。方法选取2014年12月-2018年3月于南宁市第一人民医院神经外科住院治疗的高血压脑出血患者90例作为试验组,按照Glasgow评分分为轻型亚组(12~15分)、中型亚组(9~11分)、重型亚组(3~8分),每组30例;另选取医院同期收治的原发性高血压患者30例作为对照组。试验组患者于发病后第1、7、14天采集血清标本,检测补体C3、C4水平变化,并与对照组进行比较。结果轻型亚组补体C3、C4水平在第1、7、14天与对照组比较差异无统计学意义(P>0.05);中型亚组补体C3、C4在第1天水平最低,第7天最高,第14天降至正常水平,其中补体C3第1天低于对照组(P<0.05),补体C4第1、7、14天均低于对照组(P<0.05);重型亚组补体C3、C4水平在第1、7、14天均低于对照组(P<0.05)。结论高血压脑出血患者外周血补体C3、C4水平可反映免疫功能程度,可为高血压脑出血综合治疗及改善预后提供参考依据。Objective To observe the dynamic changes of serum complement C3 and C4 in patients with hypertensive cerebral hemorrhage.Methods 90 patients with hypertensive cerebral hemorrhage hospitalized in the Department of Neurosurgery of the First People’s Hospital of Nanning from December 2014 to March 2018 were selected as the experimental group,according to Glasgow score,they were divided into light subgroup(12~15 points),medium subgroup(9~11 points),heavy subgroup(3~8 points),30 cases in each group;30 patients with primary hypertension admitted to the hospital during the same period were selected as the control group.Serum samples of patients in the experimental group were collected on days1,7,and 14 after onset to detect changes in the levels of complement C3 and C4,and compared with the control group.Results The levels of complement C3 and C4 were no significant difference between the light subgroup and the control group on days 1,7,and 14(P>0.05);The levels of complement C3 and C4 in the medium subgroup were the lowest at the 1 st day,and the highest at 7 th day,the 14 th day tended to decrease to the normal range,the level of complement C3 there was significant difference between the medium subgroup and the control group at 1 st day(P<0.05),and complement C4 was lower than the control group on days 1,7,and 14(P<0.05);The levels of complement C3 and C4 in the heavy subgroup were lower than those in the control group on days 1,7,and 14(P<0.05).Conclusion The level of complement C3 and C4 in peripheral blood can reflect the degree of immune function in patients with hypertensive intracerebral hemorrhage,which provide a reference for comprehensive treatment of hypertensive intracerebral hemorrhage patients.
