优质护理干预对保留自主神经的腹腔镜直肠癌根治术患者排尿功能恢复的影响

摘　　要：目的:探讨优质护理干预对保留自主神经的腹腔镜直肠癌根治术患者排尿功能恢复的影响。方法:采用回顾性、总结研究方法,选择2013年8月到2017年11月在我院诊治的男性直肠癌患者144例作为研究对象,根据护理方法的不同分为观察组100例与对照组44例,所有患者都给予保留自主神经的腹腔镜直肠癌根治术治疗,对照组在围手术期给予常规护理,观察组在围手术期给予优质护理干预,记录两组术后排尿功能恢复情况。结果:患者均完成手术,并没有采取中转开腹术,且两组手术时间、术中出血量比较差异无统计学意义(P>0.05)。观察组的术后排气时间、术后排便时间与术后住院时间显著少于对照组(P<0.05)。在术后14 d,在肺部感染、腹腔脓肿等并发症的发生率方面,观察组为3.0%,而对照组高达20.5%,对照组明显高于观察组(P<0.05)。术后3个月,评定患者排尿功能,观察组有70例Ⅰ级、26例Ⅱ级、4例Ⅲ级,无Ⅳ级;而对照组有8例Ⅰ级、20例Ⅱ级、10例Ⅲ级以及6例Ⅳ级,观察组显著优于对照组(P<0.05)。结论:优质护理干预在保留自主神经的腹腔镜直肠癌根治术患者中的应用能促进术后排尿功能恢复与预后康复,减少术后并发症的发生,值得临床推广应用。Objective:To explore the effects of high quality nursing intervention on the recovery of urination function in patients with laparoscopic radical resection of rectal cancer with the retention of autonomic nerves.Methods:With a retrospective summary and research methods,from August 2013 to November 2017,144 cases of rectal cancer male patients in our hospital were selected and were divided into the observation group of 100 cases and control group of 44 cases according to the different nursing methods.All the patients were treated with laparoscopic radical resection of rectal cancer with the retention of autonomic nerves.The control group was given the perioperative routine care.The observation group was given the perioperative high quality nursing intervention.The two groups of postoperative recovery of urination function were recorded.Results:All patients were successfully completed the operation without conversion to open surgery.There were no significant differences in operative time and intraoperative blood loss between the two groups(P>0.05).The postoperative exhaust time,postoperative defecation time and postoperative hospitalization time in the observation group were significantly less than those in the control group(P<0.05).The postoperative 14 d of incidence of complications in the observation group were significantly less than the control group(P<0.05).In 3 months after operation,there were 70 cases of gradeⅠ,26 cases of grade II,4 cases of grade III,0 case of grade IV of the urination function in the observaiton group,and there were 8 cases of grade I,20 cases of grade II,10 cases of grade III,6 cases of grade IV in the control group,and the observation group was significantly better than the control group(P<0.05).Conclusion:The application of high quality nursing intervention in patients with laparoscopic radical resection of rectal cancer with the retention of autonomic nerves can promote postoperative voiding function recovery and prognosis,and reduce postoperative complications,which is worthy of
