摘　　要：目的研究CD146、P16、Ki67在子宫肌瘤和富于细胞型平滑肌瘤中的表达及临床意义。方法回顾性分析2017年2月至2019年2月复旦大学附属华东医院收治的104例进行妇科手术治疗的患者,根据病理学检测结果的不同将52例富于细胞型平滑肌瘤患者纳入观察组,另选取同期来医院手术的52例子宫肌瘤患者纳入对照组。比较两组患者的CD146、P16、Ki67的表达水平和表达特点。结果CD146在两组中均有表达,其中观察组阳性率为90.4%,显著高于对照组的80.7%,差异具有统计学意义(P<0.05)。P16在两组中均有表达,其中观察组阳性率为71.2%,显著高于对照组的46.2%,差异具有统计学意义(P<0.05)。Ki67在对照组瘤旁基层组织中表达均为阴性,在肌瘤组织中部分表达,阳性率为7.69%,显著低于观察组的36.5%,差异具有统计学意义(P<0.05)。结论CD146、P16、Ki67联合监测可对富于细胞型平滑肌瘤的发生、发展做出一定的判断,从而改善患者的预后。Objective To study the expression and clinical significance of CD146,P16 and Ki67 in uterine leiomyoma and cellular leiomyoma.Methods A retrospective analysis was conducted on 104 patients who underwent gynecological surgery in Huadong Hospital,Fudan University from February 2017 to February 2019.Fifty-two patients with cellular leiomyoma were enrolled in the observation group according to the different pathological confirmations,another 52 patients with uterine leiomyoma who came to hospital for surgery at the same time were enrolled in the control group.The expression levels and characteristics of CD146,P16 and Ki67 in the two groups were compared.Results CD146 was expressed in both groups,and the positive rate in the observation group was 90.4%,which was significantly higher than 80.7% in the control group(P<0.05).The positive rate of P16 was 71.2% in the observation group,which was significantly higher than 46.2% in the control group(P< 0.05).The expression of Ki67 in the basal tissues adjacent to the tumors in the control group was negative,and the positive rate was 7.69%,which was significantly lower than 36.5% in the observation group(P< 0.05).Conclusion The combined monitoring of CD146,P16 and Ki67 can judge the occurrence and development of cellular leiomyoma and improve the prognosis of patients.
