摘　　要：目的探讨在我国农村妇女宫颈癌筛查项目中,通过TruScreen(TS)和液基细胞学(LBC)对HPV阳性人群进行进一步分流的筛查效果。方法 2018年8月~12月,对9 972例参加国家农村妇女宫颈癌筛查项目的妇女进行HPV初筛,HPV16/18阳性者进行阴道镜检查,其他12种HPV高危亚型阳性者同时进行TS和LBC检查进行分流,任一结果异常者进行阴道镜检查。通过比较TS和LBC对CIN2+和CIN3+检出率,分析两种方法对HPV检测阳性者的分流效果。结果 HPV初筛阳性率为7.54%,其中HPV16/18阳性率为2.14%,其他12种HPV高危亚型阳性率为5.41%。TS和LBC分流的异常率分别是18.74%和16.70%,两种分流策略的异常率差异无统计学意义(χ^2=0.770,P=0.380)。TS和LBC分流CIN2+检出率分别为2.41%和3.71%,差异无统计学意义(χ^2=1.532,P=0.216)。TS分流和LBC分流CIN3+检出率分别为1.11%和2.23%,差异也无统计学意义(χ^2=2.034,P=0.154)。结论 TS和LBC两种分流策略对HPV阳性人群宫颈病变的检出效果无明显差异,各地区可根据自身的实际情况选择合适的分流方法。Objective To explore the effect of human papillomavirus(HPV)primary screening and triage with TruScreen(TS)and liquid-based cytology(LBC)test in National Cervical Cancer Screening Program in Rural Areas(NCCSPRA)of China.Methods From August 2018 to December 2018,HPV test were performed on 9972 rural women who participated in NCCSPRA.Women with HPV 16/18 infection were referred for colposcopy.TS and LBC were conducted simultaneously on women with positive infection of the other 12 high-risk subtypes of HPV.Women with positive results of either test were referred to colposcopy.The triage effects of TS and LBC were analyzed by comparing their detection rates of CIN2+and CIN3+.Results The positive rate of HPV was 7.54%,HPV16/18 was 2.14%,and the other 12 high-risk types of HPV was 5.41%.As triage tests,the positive rates of TS and LBC was 18.74%and 16.70%,respectively.There was no statistical difference between the two methods(χ^2=0.770,P=0.380).The detection rate of CIN2+by TS was 2.41%and 3.71%by LBC,and there was no statistical difference(χ^2=1.532,P=0.216).The detection rate of CIN3+between TS and LBC were 1.11%and 2.23%,respectively,and also had no statistical difference(χ^2=2.034,P=0.154).Conclusions There was no difference in the screening effect of the TS and LBC triage strategies.Each region can choose the appropriate triage strategy according to its own actual situation.
