摘 要:目的探讨p16和Ki-67在不同宫颈鳞状上皮内病变中的表达情况及不同宫颈鳞状上皮内病变与高危型人乳头瘤病毒(high-risk human papilloma virus,HR-HPV)感染的关系。方法应用免疫组织化学技术检测138例宫颈病变(32例慢性宫颈炎、56例低级别鳞状上皮内病变、50例高级别鳞状上皮内病变)活检组织的p16和Ki-67表达水平,同时回顾性分析活检时HR-HPV的感染情况。结果HR-HPV感染率、p16和Ki-67阳性表达率和表达水平随着宫颈病变级别增加而逐渐增高;HR-HPV感染率与p16和Ki-67表达水平均呈正相关。结论HR-HPV的检测可用作临床宫颈病变筛查的常规检查,HRHPV及p16和Ki-67联合检测可降低宫颈病变的漏诊率,可用于指导宫颈病变的分级,提高宫颈活检诊断的准确率。Objective To explore the expression of p16 and Ki-67 in different cervical lesions and the relationship between different cervical lesions and high-risk human papilloma virus(HR-HPV)infection.Methods The expression of p16 and Ki-67 in 138 cases of cervical lesions(32 cases of chronic cervicitis,56 cases of low-grade squamous intraepithelial lesions and 50 cases of high-grade squamous intraepithelial lesions)was detected by immunohistochemical staining,and the infection of HR-HPV in cervical lesions was retrospectively observed.Results The incidence rates of HR-HPV infection and the positive rates and expression of P16 and Ki67 were gradually increased with the severity of the cervical lesions.The incidence rate of HR-HPV infection was positively correlated with the expression of P16 and Ki-67 proteins.Conclusion HR-HPV test can be used as a routine technique for screening of clinical cervical lesions.The combined detection of HR-HPV and p16,Ki-67 proteins can decrease the missed diagnosis rate of cervical lesions,guide the grading of them,and improve accuracy of cervical biopsy diagnosis.
