隆突性皮肤纤维肉瘤的CT及MRI表现与鉴别诊断

摘　　要：目的:分析隆突性皮肤纤维肉瘤的CT、MRI影像表现,以提高对该病的影像诊断水平。方法:回顾性分析在本院手术病理证实的20例隆突性纤维肉瘤的CT与MRI影像表现。结果::10例患者接受CT检查及10例患者接受MRI检查,所有患者均行增强扫描。病变均位于皮肤及皮下脂肪间隙,肿块呈单发17例,多发结节样3例,肿物缘边缘光滑(12例)或不光滑(8例),密度及信号均匀12例,CT平扫呈稍低或等密度,MRI平扫T1呈稍高或等信号,T2呈较高信号,6例侵犯相邻深筋膜及部分肌肉,增强扫描12例均匀明显强化,8例不均匀性明显强化,内见不规则无强化区或相对弱强化区,5例可见囊变区。结论::隆突性皮肤纤维肉瘤的血供极其丰富,部分呈多发结节样,常见脂肪分割征、皮肤尾征及筋膜尾征,CT及MRI对其诊断比较有特异性,有助于临床确定最佳的手术方案。Objective:To improve the diagnostic accuracy of Dermatofibrosarcoma protuberans(DFSP)by analyzing CT and MRI features of DFSP.Methods:The CT/MRI data of DFSP in 20 cases were analyzed retrospecttively,confirmed by operation and pathology.Results:10 patients underwent a CT examination and 10 patients underwent an MRI examination with enhanced scan.All lesions were located in the skin and subcutaneous fat clearance,17 cases were solitary and 3 cases were multiple nodular,edges of tumors were smooth(12 cases)or not smooth(8 cases).Masses of 12 cases of these showed isointense density and signal,all involved the skin,which showed slightly lower or isointense density on CT images,and showed slight-hyperintense or isointense signal on T1 WI and showed high signal on T2 WI,6 cases of violations of the adjacent portion of the deep fascia and muscle,after inravenous injection,masses of 12 cases enhanced homogeneously remarkably,the other showed inhomogeneous enhanced appearance,which showed irregular nonenhancement area or relatively weak enhancement area,5 cases showed visible cystic areas.Conclusion:The blood supply of DSFP is extremely rich,and some show multiple nodular,and fat division sign、skin tail sign and fascia tail sign are easy to be found,CT and MRI are more specific than other examinations,help to identify the best clinical surgery plan.
