黄连解毒汤加味对慢性牙周炎患者龈沟液IL-23、IL-1β、RANKL/OPG水平的影响

摘　　要：目的:观察黄连解毒汤加味对慢性牙周炎患者龈沟液白细胞介素-23(IL-23)、白细胞介素-1β(IL-1β)、核因子κB受体活化因子配体/骨保护素(RANKL/OPG)水平的影响。方法:选取2017年3月至2019年4月期间于我院口腔科就诊的120例慢性牙周炎患者为受试对象,采用随机数字表法随机分为观察组和对照组各60例。两组患者均给予牙周常规基础治疗,对照组予以奥硝唑分散片口服,观察组患者在对照组的基础上辅以黄连解毒汤加味治疗。比较两组患者治疗4周后的临床疗效、治疗前与治疗4周后牙周指数[牙周探诊深度(PD)、牙龈指数(GI)、菌斑指数(PLI)、牙龈出血指数(BI)、附着丧失(AL)]变化、龈沟液生化指标[白细胞介素-23(IL-23)、白细胞介素-1β(IL-1β)、核因子κB受体活化因子配体/骨保护素(RANKL/OPG)]水平变化,以及随访3个月内复发率。结果:治疗4周后,观察组患者治疗总有效率高于对照组(P<0.05);两组PLI、PD、BI、GI、AL分值均较治疗前明显下降,且观察组低于对照组(P均<0.05);两组患者龈沟液IL-23、IL-1β、RANKL/OPG水平均较治疗前明显降低,且观察组低于对照组(P均<0.05);随访3个月,观察组患者复发率低于对照组(P<0.05)。结论:黄连解毒汤加味辅助治疗慢性牙周炎疗效确切,可有效改善患者牙周状况,缓解炎症,有利于牙周组织重建。Objective:To observe the effect of Modified Huanglian Jiedu Decoction on levels of interleukin-23(IL-23),interleukin-1β(IL-1β)and receptor activator for nuclear factor-κB ligand/osteoprotegerin(RANKL/OPG)in gingival crevicular fluid for patients with chronic periodontitis.Methods:A total of 120 patients with chronic periodontitis who were admitted to department of stomatology of our hospital from March 2017 to April 2019 were selected for the study.They were randomly divided into observation group and controlled group by the random number table method,with 60 cases in each group.The two groups were given regular periodontal treatment,and the controlled group was given oral Ornidazole Dispersible Tablets while the observation group was supplemented with Modified Huanglian Jiedu Decoction on the basis of the controlled group.The clinical efficacy after 4 weeks of treatment,and periodontal indexes[periodontal probing depth(PD),gingival index(GI),plaque index(PLI),gingival bleeding index(BI),attachment loss(AL)]and biochemical markers of gingival crevicular fluid[interleukin-23(IL-23),interleukin-1β(IL-1β),receptor activator for nuclear factor-κB ligand/osteoprotegerin(RANKL/OPG)]before treatment and after 4 weeks of treatment and recurrence rate within 3 months of follow-up were compared between the two groups.Results:After 4 weeks of treatment,the total effective rate of treatment in the observation group was higher than that in the controlled group(P<0.05).The scores of PLI,PD,BI,GI and AL in the two groups were significantly lower than those before treatment,and the scores in the observation group were lower than those in the controlled group(all P<0.05).The levels of IL-23,IL-1βand RANKL/OPG in gingival crevicular fluid in the two groups were significantly lower than those before treatment,and the levels in the observation group were lower than those in the controlled group(all P<0.05).At 3 months of follow-up,the recurrence rate in the observation group was lower than that in the controlled group(P<0.05)
