白牵牛散穴位贴敷联合西药治疗乙型肝炎肝硬化腹水的研究

摘　　要：目的:观察白牵牛散穴位贴敷联合西药治疗乙型肝炎肝硬化腹水的临床疗效。方法:选取气滞湿阻型乙型肝炎肝硬化腹水患者66例,随机分为对照组和治疗组,每组33例,对照组患者给予西药常规治疗,治疗组患者在对照组治疗基础上接受白牵牛散穴位贴敷,疗程均为4周。观察治疗前后患者的中医症状积分、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆红素(TBil)、白蛋白(Alb)、凝血酶原活动度(AT)、腹部B超结果等变化,每日监测体质量、腹围、脾脏厚度、门静脉内径、24 h尿量,比较两组患者临床疗效。结果:治疗组、对照组患者临床疗效总有效率分别为93.94%、72.73%,两组比较差异有统计学意义(P<0.05)。治疗后治疗组患者中医症状积分、ALT、TBil、体质量、腹围、脾脏厚度、门静脉内径明显低于对照组(P<0.05)。Alb、AT、平均24 h尿量明显高于对照组(P<0.05)。结论:白牵牛散穴位贴敷联合西药可以明显改善乙型肝炎肝硬化腹水患者的临床症状、肝功能及凝血功能,降低门静脉压力,临床疗效显著。Objective:To observe the clinical effect of Baiqianniu powder acupoint application combined with western medicine on ascites due to hepatitis B cirrhosis.Methods:66 patients with ascites due to hepatitis B cirrhosis were randomly divided into control group and treatment group,33 cases in each group.The control group was given routine western medicine treatment,while the treatment group received Baiqianniu powder Acupoint Application on the basis of the control group.The course of treatment was 4 weeks.Before and after treatment,TCM symptom score,liver function,blood coagulation series and abdominal B-mode ultrasound were observed,and body weight,abdominal circumference and 24-hour urine volume were monitored daily.Statistical clinical efficacy.Results:The total effective rates of the treatment group and the control group were 93.94%and 72.73%,respectively.The treatment group was better than the control group,with statistical significance(P<0.05).After treatment,the scores of TCM symptoms,ALT,TB,body weight,abdominal circumference,spleen thickness and portal vein diameter in the treatment group were significantly lower than those in the control group(P<0.05).ALB,prothrombin activity and average 24-hour urine volume were significantly higher than those of the control group(P<0.05).Conclusion:Baiqianniu powder acupoint application combined with western medicine can significantly improve the clinical symptoms,liver function and coagulation function of patients with ascites due to hepatitis B cirrhosis,reduce portal vein pressure and increase plasma colloid osmotic pressure.The clinical effect is remarkable.
