长期服用利培酮、氯氮平及氯丙嗪对精神分裂症患者心电图的影响

摘　　要：目的探讨长期服用利培酮、氯氮平及氯丙嗪对精神分裂症患者心电图的影响。方法选取2015年6月至2016年6月大连市第七人民医院收治的90例首发精神分裂症患者,随机将其分为利培酮组、氯氮平组、氯丙嗪组,每组30例。观察三组患者在用药期间心电图的变化情况。结果治疗后8周,三组P、N、G评分较治疗前明显降低,差异均有统计学意义(均P<0.05);但三组治疗8周后的P、N、G评分比较,差异均无统计学意义(均P>0.05)。在治疗期间,三组患者均存在不同类型的心电图异常问题,主要表现在T波改变、传导阻滞、ST-T改变、房性早搏、窦性心率过速、室性早搏。氯氮平组的心电图异常发生率明显高于利培酮组和氯丙嗪组,差异有统计学意义(P<0.05)。结论采用利培酮、氯氮平、氯丙嗪分别对精神分裂症患者进行长期治疗均可获得显著的疗效,并能够改善患者的精神病症状,但均存在不同类型的心电图异常问题。Objective To investigate the effects of long-term administration of risperidone,clozapine and chlorpromazine on electrocardiogram(ECG)in patients with schizophrenia.Methods From June 2015 to June 2016,90 patients with first-episode schizophrenia treated in Dalian Seventh People's Hospital were randomly divided into risperidone group,clozapine group and chlorpromazine group,with 30 cases in each group.The changes of ECG in the three groups were observed.Results Eight weeks after treatment,the scores of P,N and G in the three groups were significantly lower than those before treatment(all P<0.05),but there was no significant difference in the scores of P,N and G among the three groups after 8 weeks of treatment(all P>0.05).During the treatment,there were different types of abnormal ECG problems in the three groups,such as T wave changes,conduction block,ST-T changes,atrial extrasystole,sinus tachycardia and ventricular extrasystole.The incidence of abnormal ECG in clozapine group was significantly higher than that in risperidone group and chlorpromazine group(P<0.05).Conclusion Long-term treatment of schizophrenia with risperidone,clozapine and chlorpromazine can achieve significant curative effect and improve the psychiatric symptoms,but there are different types of abnormal ECG problems.
