摘 要:目的利用三维有限元分析法在恒定静态力加载条件下,研究倾斜种植体数目的增加对种植体和骨组织应力的影响。方法在无牙下颌骨内,在46、44、42、32、34、36的位点上设计1~6号种植体,固定植入位置,以倾斜种植体数目的不同建立6组模型。模型1:1~6号种植体均倾斜0度;模型2:1号、6号种植体远中倾斜45度,其余种植体倾斜0度;模型3:1号、6号远中倾斜45度,2号、5号远中倾斜30度,其余种植体倾斜0度;模型4:1号、6号远中倾斜45度,2号、5号远中倾斜30度,3号、4号远中倾斜5度;模型5:1号、6号远中倾斜45度,2号、5号远中倾斜30度,3号、4号远中倾斜10度;模型6:1号、6号远中倾斜45度,2号、5号远中倾斜30度,3号、4号远中倾斜15度。分析6组模型的Von-Mises应力峰值及应力分布情况。结果①倾斜种植时,增加远中倾斜种植体的数目,可以降低种植体-骨组织界面的应力;②随前牙区种植体倾斜角度的增加,骨组织表面的最大应力逐渐增加;③最后2颗种植体倾斜45度植入,中间2颗种植体倾斜30度植入,前牙区种植体倾斜5度植入,可获得更小的种植体表面应力。结论下颌无牙颌固定修复倾斜种植时,增加倾斜种植体的数目可降低骨组织表面最大应力,且前牙区倾斜5度时可以获得更小的应力。Objective Under the condition of constant static force,the effect of increasing the number of inclined implants on the stress of implants and bone tissue was studied by three-dimensional finite element analysis(FEA).Methods No.1 to No.6 implants were designed at the sites of 46,44,42,32,34 and 36 in the edentulous mandible,and the sites were fixed.Six groups of models were established according to the number of inclined implants.In model 1,No.1 to No.6 implants titled 0 degrees.In model 2,No.1 and No.6 implants had a distal inclination of 45 degrees,while the rest titled 0 degrees.In model 3,No.1 and No.6 implants titled 45 degrees distally;No.2 and No.5 implants titled 30 degrees distally;other implants titled 0 degrees.In model 4,No.1 and No.6 tilted 45 degrees distally;No.2 and No.5 tilted 30 degrees distally;and No.3 and No.4 tilted 5 degrees distally.In model 5,No.1 and No.6 implants titled 45 degrees distally,No.2 and No.5 implants titled 30 degrees distally;No.3 and No.4 implants titled 10 degrees distally;in model 6,No.1 and No.6 titled 45 degrees distally,No.2 and No.5 implants titled 30 degrees distally;No.3 and No.4 implants titled 15 degrees distally.The peak Von-Mises stress and stress distribution of 6 groups of models were analyzed.Results①During tilted implantation,increasing the number of distally inclined implants could reduce the stress at the implant-bone tissue interface.②With the increase of the inclination angle of the implants in the anterior area,the maximum stress on the surface of bone tissue increases gradually.③The last two implants were implanted with an inclination of 45 degrees,and the middle two implants were implanted with an inclination of 30 degrees,and the implants in the anterior area were implanted with an inclination 5 degrees,which could obtain a smaller surface stress of the implant.Conclusion When the edentulous mandible is repaired in fixed dentures with inclined implants,increasing the number of inclined implants can reduce the maximum stress on the surface of bo
