低剂量胸部CT扫描对肺癌的诊断价值

摘　　要：目的探讨低剂量胸部CT扫描对肺癌的诊断价值。方法选取2018年7月至2019年5月河津市人民医院收治的90例疑似肺癌患者作为研究对象,回顾性分析其临床资料,所有患者分别进行低剂量胸部CT扫描及常规胸部CT扫描。记录90例疑似肺癌患者低剂量胸部CT扫描和常规胸部CT扫描结果,所得数据与病理检查或随访确诊结果进行对比,同时记录两种检查方法的影像学特征、容积CT剂量指数(CTDIvol)、剂量长度乘积(DLP)等相关指标。结果90例疑似肺癌患者各项检查完成率均为100.00%。低剂量胸部CT对肺癌的诊断敏感性为94.87%,特异性为83.33%;常规胸部CT对肺癌的诊断敏感性为96.15%,特异性为91.67%,两种检查方法比较差异未见统计学意义(P>0.05)。90例疑似肺癌患者的常规胸部CT检查与低剂量胸部CT检查的影像学特征比较,差异未见统计学意义(P>0.05)。90例疑似肺癌患者经常规胸部CT检查过程中CTDIvol、DLP水平均高于低剂量胸部CT检查(P<0.05)。结论应用低剂量胸部CT扫描对诊断肺癌具有重要价值,且此法能够有效提高患者检查安全性。Objective To investigate the diagnostic value of low-dose chest computed tomography(CT)scan for lung cancer.Methods A total of 90 suspected lung cancer patients admitted to Hejin People’s Hospital from July 2018 to May 2019 were selected as the research objects,and their clinical data were analyzed retrospectively.All patients were scanned by low-dose chest CT and conventional chest CT respectively.The results of low-dose chest CT scan and routine chest CT scan of 90 patients with suspected lung cancer were recorded.The data were compared with pathological examination or follow-up diagnosis results,meanwhile,the imaging characteristics,volume CT dose index(CTDIvol)and dose length product(DLP)were recorded.Results The complete rate of all examinations of 90 patients with suspected lung cancer was 100.00%.The sensitivity and specificity of low-dose chest CT were 94.87%and 83.33%respectively,the sensitivity and specificity of routine chest CT were 96.15%and 91.67%respectively,and there was no significant difference between the two methods(P>0.05).There was no significant difference in the imaging characteristics between the conventional chest CT and low-dose chest CT in 90 patients with suspected lung cancer(P>0.05).The level of CTDIvol and DLP in routine chest CT examination were higher than those in low-dose chest CT in 90 suspected lung cancer patients(P<0.05).Conclusions Low-dose chest CT scan is of great value in the diagnosis of lung cancer,and the method can effectively improve the examination safety.
