18F-FDG符合线路SPECT-CT显像在原发性神经中枢淋巴瘤诊断中的应用

摘　　要：目的探讨氟-18脱氧葡萄糖(18F-FDG)符合线路单光子发射计算机断层成像术(SPECT-CT)在原发性神经中枢淋巴瘤(PCNSL)诊断中的价值。方法回顾性分析2016年2月至2019年2月于开封市中心医院接受检查并确诊为PCNSL的60例患者的临床资料,所有患者均接受手术治疗,并于术前接受18F-FDG符合线路SPECT-CT检查。根据术后病理检查结果,比较同机CT图像与融合图像对PCNSL的检出率。结果60例患者经术后病理诊断,56例证实为PCNSL。56例PCNSL患者中,18F-FDG符合线路SPECT-CT检出阳性52例,检出率为92.86%;CT检出阳性40例,检出率为71.43%,两种检查方式的检出率比较差异有统计学意义(P<0.05)。18F-FDG符合线路SPECT-CT检查方法的敏感度、特异性(0.930、0.750)均高于CT检查(0.929、0.059);18F-FDG符合线路SPECT-CT检查方法与病理检查结果的一致性(Kappa=0.503,>0.45)高于CT检查(Kappa=0.014,<0.20)。结论18F-FDG符合线路SPECT-CT检查能有效检出PCNSL,在临床诊断中的灵敏度及特异性更高。Objective To investigate the diagnostic value of fluorine-18 deoxyglucose(18F-FDG)coincidence line single-photon emission computed tomography(SPECT-CT)for primary central nervous system lymphoma(PCNSL).Methods The clinical data of 60 patients who had physical examination and were confirmed as PCNSL in Kaifeng Central Hospital from February 2016 to February 2019 were retrospectively analyzed.All patients were treated by surgery,and they received 18F-FDG coincidence SPECT-CT examination before surgery.According to the results of postoperative pathological examination,the detection rates of PCNSL by computed tomography(CT)images and fusion images of the same machine were compared.Results According to pathological diagnosis,56 of 60 patients were confirmed as PCNSL.Among the 56 patients,52 cases were determined as positive by 18F-FDG coincidence SPECT-CT,with the detection rate of 92.86%,40 cases were determined as positive by CT,with the detection rate of 71.43%;there was a significant difference in the detection rate between the two methods(P<0.05).The sensitivity and specificity of the 18F-FDG coincidence SPECT-CT method were 0.930 and 0.750,respectively,which were higher than the 0.929,0.059 of the CT in the diagnosis of PCNSL.The consistency rate of 18F-FDG coincidence SPECT-CT with pathological examination(Kappa=0.503,>0.45)was higher than that of CT with pathological examination(Kappa=0.014,<0.20).Conclusions 18F-FDG coincidence SPECT-CT is effective in the diagnosis of PCNSL,with higher sensitivity and specificity.
