摘  要:目的探讨肺癌患者医院感染与白细胞介素-27(IL-27)基因多态性及外周血微小RNA(microRNA-31和microRNA-150)表达相关性。方法选择2017年1月-2018年12月于杭州师范大学附属医院治疗的肺癌患者483例为研究对象,根据是否发生医院感染分为感染组64例与未感染组419例。分离鉴定肺癌患者医院感染的病原菌。采用实时荧光定量PCR法检测患者IL-27基因多态性及外周血microRNA-31和microRNA-150表达。结果肺癌住院患者483例,感染患者64例,感染率为13.25%,以呼吸道感染为主。共培养分离病原菌73株,其中革兰阴性菌48株占65.75%,以肺炎克雷伯菌和铜绿假单胞菌为主;革兰阳性菌23株占31.51%,以金黄色葡萄球菌为主;真菌2株占2.74%。感染组IL-27964基因型AA分布率为56.25%(36/64)高于未感染组,而GG分布率为31.25%(20/64)低于未感染组(P<0.05);两组IL-272905基因型TT、TG和GG基因型分布比较差异无统计学意义。感染组IL-27964等位基因A分布率为62.50%(40/64)高于未感染组(P<0.05),而IL-272905等位基因T和G分布比较差异无统计学意义。感染组外周血microRNA-31和microRNA-150表达分别为(2.38±0.35)和(1.63±0.27)高于未感染组(P<0.05)。结论肺癌患者医院感染外周血microRNA-31和microRNA-150表达明显上调,认为IL-27964 A基因为肺癌感染易感基因。OBJECTIVE To investigate the correlation between nosocomial infection,interleukin-27(IL-27)gene polymorphism and the expression of microRNA-31 and microRNA-150 in peripheral blood of lung cancer patients.METHODS Totally 483 patients with lung cancer admitted to Affiliated Hospital of Hangzhou Normal University from Jan.2017 to Dec.2018 were selected as the study subjects,and were divided into 64 cases of infection group and 419 cases of non-infection group according to the occurrence of nosocomial infection.Pathogenic bacteria in patients with nosocomial infection of lung cancer were isolated and identified,and the real-time fluorescence quantitative PCR were used to detect the polymorphism of IL-27 gene and the expression of microRNA-31 and microRNA-150 in peripheral blood.RESULTS There were 483 hospitalized patients with lung cancer and 64 infected patients with an infection rate of 13.25%,mainly respiratory infection.A total of 73 strains of pathogenic bacteria were isolated,of which 48 strains of gram-negative bacteria accounted for 65.75%,mainly Klebsiella pneumoniae and Pseudomonas aeruginosa;23 strains of gram-positive bacteria accounted for 31.51%,mainly Staphylococcus aureus;and 2 stains of fungi accounted for 2.74%.The distribution rate of AA in il-27964 genotype in the infection group was 56.25%(36/64),significantly higher than that in the non-infection group,while the distribution rate of GG was 31.25%(20/64),significantly lower than that in the non-infection group(P<0.05).The distribution rate of IL-27964 allele A in the infected group was 62.50%(40/64)significantly higher than that in the uninfected group(P<0.05),but there was no significant difference in the distribution of TT,TG and GG genotype of IL-272905 genotype between two groups.The distribution rate of IL-27-allele A in the infection group was 62.50%(40/64),significantly higher than that in the non-infectiong group,while there were no significant difference between the distribution of T and G in the IL-272905 allele.The expression of micror
