夹板式粘结桥治疗牙周炎伴牙列缺损的效果观察

摘　　要：目的:探讨夹板式粘结桥治疗牙周炎伴牙列缺损的效果。方法:抽取2015年1月至2018年6月山西大同煤矿集团总医院收治的牙周炎伴牙裂缺损患者120例,采用随机数字表法分为研究组和对照组,每组60例,研究组采用夹板式粘结桥修复治疗、对照组采用传统修复治疗。比较两组治疗效果及手术前后龈沟出血指数(SBI)、牙周附着丧失(CLA)、龈沟液细胞因子检测值。结果:术后6、12个月,研究组牙周SBI、CLA值低于对照组(F=10.382、6.773、15.228、8.852,P=0.001、0.001、0.001、0.001)。术后6个月,研究组白细胞介素-8、肿瘤坏死因子-α、白细胞介素-1β、β2防御素水平低于对照组(t=-4.233、-3.755、-2.751、-3.890,P=0.001、0.001、0.007、0.001)。术后12个月,研究组显效率(55.00%,33/60)高于对照组(35.00%,21/60),差异有统计学意义(Z=-2.265,P=0.024)。结论:夹板式粘结桥治疗牙周炎伴牙列缺损能够更有效地改善牙龈出血和牙齿松动症状,促进牙周组织恢复,提高修复效果。Objective To investigate the effects of splint bonded bridge on periodontitis with dental cleft defect.Methods A total of 120 patients with periodontitis and dental cleft defect admitted to General Hospital of Datong Coal Mining Group from January 2015 to June 2018 were selected.They were divided into study group and acontrol group by random number table method,with 60 cases in each group.The patients in the study group was repaired with splint bonded bridge,and the patients in the control group was treated with traditional repair.The treatment effect and the sulcus bleeding index(SBI),periodontal attachment loss(CLA),and cytokines of gingival crevicular fluid before and after surgery were compared between the two groups.Results Six and twelve months after surgery,the periodontal SBI and CLA values in the study group were lower than those in the control group(F=10.382,6.737,15.228,8.852,P=0.001,0.001,0.001,0.001).The levels of interleukin-8,tumor necrosis factor-α,interleukin-1β,andβ2 defensin in the study group six months after operation were lower than those in the control group(t=-4.233,-3.755,-2.751,-3.890,P=0.001,0.001,0.007,0.001).Twelve months after surgery,the efficiency of the the study group was 55%(33/60),which was higher than 35%(21/60)of the control group,and the difference was significant(Z=-2.265,P=0.024).Conclusions The splint bonded bridge on periodontitis with dental cleft defect can effectively improve the symptoms of gum bleeding and tooth loosening,promote the recovery of periodontal tissue,and improve the repair effect.
