摘 要:[目的]探索钆塞酸增强MRI中肝细胞摄取率与吲哚菁绿滞留率(ICG R15)的相关性以及在预测肝功能和并发症诊断中的价值。[方法]选取2017年5月~2019年3月,选取80例慢性肝病或肝硬化的患者进行MRI检查,获得给予钆塞酸前后的T1值,测定吲哚菁绿滞留率(ICG R15),比较并评估ICG R15与MRI各参数[肝脏对比前后T1值、肝细胞摄取率(肝细胞摄取造影剂的量)、肝脏体积、胆总管的增强程度]之间的相关性。比较受试者操作特性曲线下的面积进行评估肝细胞摄取率在预测肝功能和并发症的诊断的性能。[结果]肝细胞摄取率与ICG R15呈负相关,在慢性疾病或Child-Pugh A级肝硬化患者中,ICG R15≤20%的患者肝细胞摄取率高于ICG R15>20%的患者(P<0.001)。肝细胞摄取率比其他MRI参数在检测ICG R15>20%的患者上表现出更好的性能。[结论]肝细胞摄取率与吲哚青绿保留率呈负相关,能够定量评估肝功能,在识别并发症上具有良好的诊断性能。[Objective]To explore the correlation between hepatocyte uptake ratios and indocinine green retention rate(ICG R15)in gadolinium acid enhanced MRI and its value in predicting liver function and complication diagnosis.[Methods]Between May 2017 to March 2019,80 patients with chronic liver disease or cirrhosis underwent MRI,T1 values were obtained before and after gadolinium acid administration,Liver function was quantitatively assessed by using the indocyanine green retention test(ICG R15),compare and evaluate ICG R15 and MRI parameters[precontrast and postcontrast T1 values of the liver,hepatocyte uptake ratio(the amount of contrast media solely taken into hepatocytes),liver volume,degree of enhancement at the common bile duct].The area under the receiver operating characteristic curve was compared to evaluate the ability of hepatocyte uptake ratios to predict the diagnosis of liver function and complications.[Results]Hepatocyte uptake ratio showed a negative correlation with ICG R15. In participants with chronic liver disease or Child-Pugh class A,those with ICG R15 of 20% or less showed higher hepatocyte uptake ratio than those with ICG R15 greater than 20%.Hepatocyte uptake ratios demonstrated better performance for helping to detect ICG R15 greater than 20% than other MRI parameters.[Conclusion]Hepatocyte uptake ratios are negatively correlated with indocyanine green retention test,which can quantitatively evaluate liver function and have good diagnostic performance in identifying complications.
