摘 要:目的分析神经纤维瘤病Ⅰ型(NF-Ⅰ)儿童脑实质异常T2 WI高信号的表现特点.方法回顾性分析25例确诊为NF-Ⅰ儿童脑实质异常T2 WI高信号.结果25例患儿脑实质均出现异常T2 WI高信号,各部位的发病率依次为:小脑18例(72%)、脑干16例(64%)、基底节14例(56%)、丘脑13例(52%)、海马3例(12%).双侧发病者:小脑16例(89%)、基底节10例(71%)、丘脑13例(100%)、海马3例(100%).边界清楚者:基底节14例(100%)、丘脑1例(8%)、小脑12例(67%)、脑干6例(37%).边界不清者:丘脑12例(92%)、小脑4例(33%)、脑干10例(63%)、海马3例(100%).病灶数目与患儿年龄无相关性(r=0.246,P=0.235).结论T2 WI高信号是NF-Ⅰ儿童脑实质常见影像学表现,多见于小脑、脑干、基底节、丘脑,且多为双侧,基底节病变多边界清楚,丘脑病变多边界不清,认识T2 WI高信号表现特点有助于NF-Ⅰ的诊断及鉴别诊断.Objective To analyzxe the chauracteristics of abnormal T2WI hyperintensity in brain parenchyma of children with neurofibromatosis typeⅠ(NF-Ⅰ).Methods The abnormal T2WI bhyperintensity in brain parenchyma of 25 children with NF-Ⅰwere analyzed retrospectively.Results Abnormal T2WI hyperintensity was observed in the brain parenchyma of all the 25 children with NF-Ⅰ.The morbidity of each part was as follows:cerebellum in 18 cases(72%),brainstem in 16 cases(64%),basal ganglia in 14 cases(56%),thalamus in 13 cases(52%)and hippocampus in 3 cases(12%).Bilateral cases:cerebellum in 16 cases(89%),basal ganglia in 10 cases(71%),thalamus in 13 cases(100%),hippocampus in 3 cases(100%).Clear boundary:basal ganglia in 14 cases(100%),thalamus in 1 case(8%),cerebellum in 12 cases(67%),brain stem in 6 cases(37%).There were 12 cases of thalamus(92%),4 cases of cerebelum(33%),10 cases of brainstem(63%),3 cases of hippocampus(100%).There was no correlation between the number of lesions and the age(r=0.246,P=0.235).Conclusion T2WI hyperintensity is a common manifestation in brain parenchyma of children with NF-Ⅰ.T2WI hyperintensity tends to be in cerebellum,brainste,basal ganglia and thalami and most lesions were bilateral.Tr:WI hyperintensity lesions in basal ganglia almost have clear border,while T2WI hyperintensity lesions in thalamic have unclear border.Recognizing the characteristics of T2WI hyperintensity is helpful for the diagnosis and differential diagnosis of NF-Ⅰ.
