摘  要:目的 利用功能磁共振(fMRI)比较观察脑梗死后抑郁(PSD)患者、脑梗死后无抑郁(NPSD)患者及健康志愿者(HC)后扣带回(PCC)与全脑其他脑区功能连接改变情况,评估脑梗死后抑郁患者默认网络与全脑其他网络的功能情况,探索脑梗死患者抑郁的形成及发展原因.方法 选择2013年9月至2016年12月期间首次发作脑梗死患者,对PSD组(18例)、NPSD组(19例)以及HC组(13例)进行静息态fMRI成像扫描.以后扣带回(PCC)为感兴趣区(ROI),进行静息态脑功能连接分析.研究各组间被试者感兴趣区静息态功能连接(FC)值的差异.结果 HC组PCC与两侧前扣带回(ACC)、右侧内侧眶额叶及右侧上内侧额叶的功能连接较NPSD组增强.HC组PCC与两侧ACC、右侧内侧眶额叶、左侧丘脑、左侧海马、左侧距状回、小脑及小脑蚓部的功能连接较PSD组增强.PSD组PCC与中央前回的功能连接较HC组增强.PSD组PCC与左侧颞中回及左侧顶下小叶的功能连接较NPSD组增强.结论 PSD患者的默认网络及边缘系统均有一定程度的功能紊乱.在脑梗死患者中,抑郁的发生发展与非脑梗死抑郁患者具有一定的一致性,同时,也具有其特殊性,对于脑梗死患者的中枢机制需进一步研究.Objective To analysis the Rinctional connectivity cf posterior cingulate cortex(PCC)with other brain regions in patients with post-stroke depression(PSD),without post-stroke depression and the healthy controls by using resting-state fiinctional magnetic resonance imaging(fMRI).Methods Eighteen patients with PSD,19 patients without PSD and 13healthy controls were recruited in this study.The posterior cingulate cortex(PCC)was selected as region of interest(ROI),and the Rinctional connectivity between PCC and the rest of other brain regions was analyzed.Results Non-PSD group showed decreased Rinctional connectivity between PCC and both sides anterior cingulate cortex(ACC),right side medial orbitofrontal lobe and superior medial frontal lobe when compared with HC group.PSD group showed decreased fiinctional connectivity between PCC and both sides ACC,right side medial orbitofrontal lobe,left side thalamus,hippocampus,calcarine,cerebellum and verrnis of cerebellum when compared with HC group.PSD group showed increased functional connectivity between PCC and precentral lobe when compared with HC group.PSD group showed increased functional connectivity between PCC and left side middle temporal lobe and inferior parietal lobe when compared with Non-PSD group.Conclusion The default mode network participants in the progress of PSD.In patients with stroke,the occurrence and development of depression are in some extent consistent with those of non-infarct depression patients,and at the same time,it has its particularity.Further research is needed on the central mechanism of infarction patients.
