摘　　要：目的 总结卵巢成熟囊性畸胎瘤伴蒂扭转的CT特征,探讨CT对其诊断价值.方法 回顾性分析12例经病理证实的卵巢成熟囊性畸胎瘤伴蒂扭转的CT征象.结果 12例卵巢成熟性畸胎瘤伴蒂扭转在CT上均表现为脂肪密度为主的混杂密度的囊性肿块和实性双肿块.8例囊壁局限性增厚,4例弥漫性增厚.实性肿块呈椭圆形、绳索状或腊肠状软组织肿块,大部分呈等、稍低或等低混杂密度,其中1例呈不均匀高密度.子宫向扭转侧移位10例,向对侧移位2例.结论 卵巢成熟囊性畸胎瘤伴蒂扭转具有盆腔脂肪-实性双肿块的CT特征,正确诊断有助于临床早期干预,减少并发症.CT对卵巢成熟囊性畸胎瘤伴蒂扭转具有较高的临床价值.Objective To summarize CT features of pedicle torsion of ovarian mature cystic teratomas,and to explore the value of CT in its diagnosis.Methods CT signs of 12 cases of ovarian mauire cystic teratomas with pedicle torsion,confirmed by pathology,were retrospectively analyzed.Results All the 12 cases of pedicle torsion of ovarian mature cystic teratomas were diagnosed as pelvic cystic-solid double mass on CT,and pelvic cystic mass on CT manifested as a mixed density cystic mass with fat density as its main component.Cystic wall demonstrated local thickening in 8 cases and diffiise thickening in 4 cases.The solid mass appeared as the oval,cord-like or sausage-like mass,which presented as iso-density,slight hypo-density or mixed density with iso-density and hypo-density in all cases except 1 case of heterogeneous hyper-density.Uterine deviation to the twisted side was seen in 10 cases,but uterine deviation to the opposite side was seen in 2 cases.Conclusion Pedicle torsion of ovarian mature cystic teratomas has the characteristic CT findings of pelvic fat-solid double mass.The correct diagnosis of ovarian mature cystic teratomas with pedicle torsion can contribute to rational intervention and reduction of complications in the early stage of the disease.CT has great value for diagnosis in pedicle torsion of ovarian mature cystic teratomas.
