态靶辨证在痰热互结型代谢综合征的运用——小陷胸汤加威灵仙、红曲、三桑

摘　　要：代谢综合征是一组以腹型肥胖、胰岛素抵抗、高血压、高血脂为临床特征的代谢紊乱证候群。目前尚无针对性治疗药物,临床常推荐改变生活方式以调节代谢紊乱,中医药以整体观念从多靶点调整多系统代谢,疗效显著。仝小林院士从脾瘅论治代谢综合征,认为其病因起于过食肥甘厚味,中满内热是其核心病机,根据肥胖-代谢综合征-并发症(心脑血管病)的动态发展过程,可分为郁热虚损四个阶段,其中痰热互结型是热阶段最典型的证候。痰热互结型代谢综合征以腹型肥胖、胸闷脘痞为辨证要点。治则在以小陷胸汤调其痰热之态的基础上,配伍威灵仙、红曲、三桑(桑叶、桑枝、桑白皮)分别减低血尿酸、血脂、血糖,态下分证,态靶结合。Metabolic syndrome(MetS) is defined as a pathologic condition characterized by abdominal obesity,insulin resistance,hypertensionand hyperlipidemia.At present,there is no targeted therapeutic drug,and the therapeutic lifestyle changes is often recommended to regulate metabolic disorders in the clinic.Traditional Chinese medicine has an effect on the treatment of MetS via adjusting multi-system metabolism from multiple targets with a holistic theory.Professor TONG Xiaolin could treat MetS from Pidan.He thinks that MetS is leaded by over-eating fat and sweet,and abdominal fullness and interior heat is the root pathogenesis.According to dynamic progress of obesity-MetS-complication(cardio-cerebral vascular disease),MetS could be divided into four stages(depression,heat,deficiency,damage).The phlegm and heat mutual syndrome is the most typical syndrome in heat-state,which is characterized by abdominal obesity and chest tightness.The treatment method is adjusting phlegm and heat state with Xiaoxianxiong Decoction,and combining with Weilingxian(Radix clematidis) to decrease urinary acid,using Hongqu(Red yeast rice) to decrease lipid and Sansang [Sangye(mulberry leaf),Sangzhi(mulberry twig) and Sangbaipi(cortex mori radicis)]to reduce glucose.
