基于《内经》探析醒髓汤治疗脊髓损伤虚实夹杂证

摘　　要：脊髓损伤在临床上属于复杂的骨科疾患,其治疗周期长且预后差,对患者的身心健康及经济状况造成了严重影响。《内经》将其归为"痿证"范畴,并认为其中医辨证分型存在虚实之别,因此,在中医药理论的指导下,该院自拟经验方——醒髓汤并用于脊髓损伤的治疗,目前已经取得了一定的临床和理论研究基础。文章基于《内经》深入探讨了脊髓损伤的中医病因病机,进一步纵观分析了醒髓汤的组方配伍与用药特点,阐明了其适用于脊髓损伤中医辨证属虚实夹杂证之脾胃虚弱兼夹湿热、瘀血型。同时结合醒髓汤的现代分子机制研究和单味中药及其提取物的相关药理学等研究,全面探析了醒髓汤治疗脊髓损伤的机制。全文分别从中医和西医的角度进行论述,极大丰富了醒髓汤用于脊髓损伤修复的理论依据,为醒髓汤的临床研究和基础研究奠定了扎实的理论基础。Spinal cord injury is a complicated orthopaedic disease in clinical practice,whose treatment cycle is long and the prognosis is unfavourable,and also has a serious impact on the physical and mental health and economic status of patients.Neijing classifies it as "flaccidity syndrome",and holds that there are excess and deficiency syndromes in traditional Chinese medicine(TCM) syndrome classification.Therefore,under the guidance of the theory of traditional Chinese medicine,we have drawn up the empirical formula Xingsui Decoction and applied it in the treatment of spinal cord injury,which has already obtained certain clinical and theoretical research basis.In this paper,the etiology and pathogenesis of spinal cord injury were deeply discussed based on Neijing,and the compatibility and characteristics of the experiential prescription Xingsui Decoction were further analyzed.And Xingsui Decoction was applied to spleen and stomach weak with heat-dampness and blood stasis syndrome in spinal cord injury.At the same time,combined with the modern molecular mechanism and the pharmacology of single Chinese medicine and its extract of Xingsui Decoction,the mechanism of Xingsui Decoction in the treatment of spinal cord injury were comprehensively analyzed.This paper discussed from the perspectives of traditional Chinese medicine and western medicine,greatly enriched the theoretical basis for the application of Xingsui Decoction in the repair of spinal cord injury,and laid a solid theoretical foundation for the clinical research and basic research of Xingsui Decoction.
