通腹健脾法对早期结直肠癌腹腔镜治疗围手术期的应用价值研究

摘　　要：目的探讨通腑健脾法对早期结直肠癌腹腔镜治疗围手术期的应用价值研究。方法选取2017年5月—2018年6月本院择期行早期结直肠癌腹腔镜手术患者68例,经医学伦理委员会审核,随机分为对照组(n=34例)和治疗组(n=34例),对照组采用快速康复理念,治疗组在上述基础上联合通腑健脾法治疗,持续治疗7 d。比较两组患者治疗前、治疗第3天及治疗第7天时采血测定免疫功能、营养代谢指标及炎症因子等,同时评价药物的安全性。结果对照组在首次排气时间、肠道通气时间、首次下地时间及住院时间上高于治疗组(P<0.05)。两组在不同时间点CD3^+、CD4^+、CD4^+/CD8^+、TP、PA、ALB、TF、IL-6、TNF-α、CRP差异存在统计学意义(P<0.05),与治疗前比,两组治疗第3天、第7天CD3^+、CD4^+、CD4^+/CD8^+升高,TP、PA、ALB、TF升高,IL-6、TNF-α、CRP降低(P<0.05);与对照组比,治疗组治疗第3天、第7天CD3^+、CD4^+、CD4^+/CD8^+较高,TP、PA、ALB、TF较高,IL-6、TNF-α、CRP较低(P<0.05)。结论通腑健脾法对于早期结直肠癌腹腔镜手术围手术期,可加速胃肠功能恢复,提高免疫功能及营养代谢,具有较高的应用价值。Objective To explore the promoting the blood circulation of abdomen and tonifying spleen method in the perioperative period of early colorectal cancer laparoscopic surgery in perioperative period of early colorectal cancer laparoscopic surgery.Methods Sixty-eight patients with early colorectal cancer underwent laparoscopic surgery from 2017.5 to 2018.6 were randomly divided into control group(n=34)and the study group(n=34)according to the principle of case control.The control group adopted the concept of rapid rehabilitation,and the study group was treated more with methords of promoting the circulation of abdomen and tonifying spleen.Immune function,nutritional metabolic indicators and inflammatory factors were observed while the safety of drugs were compared in two groups before treatment,3 and 7 days after treatment.Results Compared with those of the study group,the first exhaust time,intestinal ventilation time,the first time and the hospitalization time were longer in the control group(P<0.05).There were significant differences in CD3^+,CD4^+,CD4^+/CD8^+,TP,PA,ALB,TF,IL-6,TNF-αand CRP between the two groups at different time points(P<0.05).Compared with those before treatment,levels of CD3^+,CD4^+and CD4^+/CD8^+increased in two groups at the 3rd and 7th day,levels of TP,PA,ALB and TF increased,levels of IL-6,TNF-α and CRP decreased(P<0.05).Compared with those of the control group,the levels of CD3^+and CD4^+/CD8^+were higher,levels of TP,PA,ALB and TF were higher,levels of IL-6,TNF-αand CRP were lower(P<0.05).Conclusion Promoting the blood circulation of abdomen and tonifying spleen method in the perioperative period of early colorectal cancer laparoscopic surgery can accelerate the recovery of gastrointestinal function,improve the immune function and nutrient metabolism,and has high application value.
