摘  要:目的探讨纳米碳在甲状腺乳头状癌隐匿性侧颈转移淋巴结清扫术中的临床应用价值。方法60例临床淋巴结阴性甲状腺乳头状癌在术中注射纳米碳,切除黑染侧颈淋巴结送冰冻病理,根据病理情况决定淋巴结清扫范围。结果60例患者术中侧颈淋巴结黑染58例,冰冻病理证实淋巴结阳性12例,行侧颈淋巴结清扫术12例。术后随访未见局部复发。结论临床N0甲状腺乳头状癌通过术中注射纳米碳示踪可增加侧颈隐匿性转移淋巴结检出率,减少阳性淋巴结漏诊率,提高手术效果。Objective To investigate the effect of nano-carbon suspension in the dissection of occult lateral cervical metastatic lymph nodes in papillary thyroid carcinoma.Methods Sixty cases of clinically node-negative papillary thyroid cancer were injected with nano-carbon during the operation.Lateral lymph node dissection was performed according to the frozen pathology of the coloration of the lymph nodes.Results During the operation,58 cases of lateral cervical lymph nodes were black stained,and 12 cases of positive lateral lymph nodes were confirmed by frozen pathology.Then,the 12 cases of positive lymph nodes underwent lateral cervical lymph node dissection.Postoperative local recurrence was not found during follow-up.Conclusion Clinically node-negative papillary thyroid carcinoma using the nano-carbon injection lymph node tracing technique can improve the positive detection rate of occult lateral cervical lymph nodes,reduce the misdiagnosis rate of positive lymph nodes,and improve the surgical effect.
