外伤性无晶状体眼无缝线巩膜层间人工晶状体固定术的效果观察

摘　　要：目的评价外伤性无晶状体眼无缝线巩膜层间人工晶状体固定术的效果。方法回顾性分析山东中医药大学附属眼科医院2018年1月至2019年5月就诊的外伤性无晶状体眼37例(37只眼)的临床资料。患者年龄范围14~57岁,平均(44.27±7.24)岁。均进行无缝线巩膜层间人工晶状体固定术。术后观察患者最佳矫正视力(logMAR)、眼压变化、并发症及人工晶状体位置情况。术后随访3~18个月,平均(7.7±1.3)个月。结果术后BCVA 0.42±0.25优于术前的0.60±0.32(t=-10.014,P=0.010)。术后眼压11.2~20.3 mmHg,平均(15.3±2.1)mmHg(1 mmHg=0.133 kPa)。术后4例(10.81%)玻璃体积血,9例(24.32%)角膜轻度水肿。随访期间未发生人工晶状体偏位、脱位、襻暴露、感染、角膜内皮细胞功能失代偿、继发性青光眼或视网膜脱离等并发症。结论无缝线巩膜层间人工晶状体固定术治疗外伤性无晶状体眼操作简单,效果好,并发症少。Objective To evaluate the efficacy of intraocular lens scleral fixation without sutures for the treatment of traumatic aphakia.Methods The clinical data of 37 eyes of 37 cases with traumatic aphakia from Jan.2018 to May 2019 in the Affiliated Eye Hospital of Shandong University of Traditional Chinese Medicine were analyzed retrospectively.The average age was(44.27±7.24)years,ranging from 14 to 57.The intraocular lens were implanted in all patients with sutureless scleral fixation.Postoperative follow-up time was 3-18 months,with an average of(7.7±1.3)months.The best corrected visual acuity(logMAR),IOP,complication and location of IOL after operation were observed.Results The postoperative BCVA was 0.42±0.25,which was better than the 0.60±0.32 preoperatively.The difference was statistically significant(t=-10.014,P=0.010).The postoperative intraocular pressure was 11.2-20.3 mmHg,with an average of(15.3±2.1)mmHg(1 mmHg=0.133 kPa).After operation,there were 4 cases(10.81%)with vitreous hemorrhage,and 9 cases(24.32%)with mild corneal edema.There were no complication such as dislocation of intraocular lens,exposure,infection,functional decompensation of corneal endothelial cells,secondary glaucoma or retinal detachment during the follow-up.Conclusion Sutureless scleral intraocular lens fixation surgery is simple,has good efficacy and less complications for the treatment of traumatic aphakia.
